Position Summary
The quality assurance nurse is responsible for review of all clinical records regarding compliance with regulation, OASIS, quality of clinical documentation, coordination of the multidisciplinary care plan, and development of the plan of care signed by the physician, including identification and implementation of staff education and training to achieve superior clinical outcomes and quality of care.
Key Responsibilities
- Completes clinical record review at start of care, resumption, recert, and discharge for complete and accurate documentation, development of appropriate discipline-specific care plan, accurate physician orders, and coordination of care and compliance with Medicare and state regulations.
- Completes reviews within agency-defined timeframe.
- Demonstrates understanding of current Medicare guidelines and ensures that clinical records are compliant with CoPs. Maintains agency in survey-ready status.
- Demonstrates proficiency in use of the EMR (Kinnser) and other available web-based programs.
- Provides expertise in OASIS in record review and staff education.
- Collaborates with coder to ensure accuracy of OASIS and develop educational strategies to optimize outcomes.
- Maintains proficiency in OASIS dataset and data capture best practices, according to industry guidelines and CMS instructions.
- Assures agency compliance with OASIS requirements.
- Completes record review according to agency-defined parameters and timeframes.
- Compiles findings and creates reports as assigned.
- Facilitates the activities of the QAPI committee.
- Ability to complete home health skilled nursing visits as required to meet patient care needs of the agency.
- Serves as back up DON.
Qualifications
- A Registered Nurse with current licensure to practice professional nursing in the State of Illinois.
- A bachelor’s degree in Nursing from an accredited program by the National League for Nursing is required.
- Minimum 3 years recent Medicare home health experience as QA nurse or RN case manager.
- Minimum 2 years’ experience in skilled nursing clinical assessment and OASIS completion.
- Proficient in point of care and electronic medical record system.
