JOB SUMMARY: Ensures the quality and safe delivery of Haven Home Health services and assists with directing, supervising and evaluating the activities of the clinical area.
DUTIES AND RESPONSIBILITIES:
Assist in the development of programs and functions for the Haven Home Health as they relate to clinical services
- Supervise responsibilities within the province of their licensing
- Participate in QAPI activities and all activities relevant to the professional services furnished
- Evaluating own job performance and utilizing available help in planning for professional growth
- Assist in evaluating the performance of other individuals in the division in relation to established standards and the individual’s professional development
- Implementing the care plan as it is written
- Evaluating effectiveness of care plan and make recommendations for revisions as appropriate and in relation to authorization processes as directed by Administrator/DON
- Evaluating total nursing services in relation to Haven Home Health goals
- Participate in periodic meetings with the Administrator/DON to ensure optimum communication interdepartmentally and to discuss any mutual or Haven Home Health problems
- Assist in the review of all logs including but not limited to infection control, patient complaints and incident reports on at least a quarterly basis, investigates problems/potential problems noted and offers recommendations for resolution/change as directed by Administrator/DON
- Available at all times during operating business hours either on site or by telecommunication
- Report any suspected violations of the privacy practice to the privacy official as soon as the breach/possible breach is identified
- Maintain self in a professional manner at all times and situations
- Maintain strict confidentiality of all patient, employee and Haven Home Health information
- Participate in all Haven Home Health in-service programs
- Provide for the emotional and physical comfort and safety of client taking into consideration their rights and cultural background
- Receive and transcribe physician orders accurately
- Notify supervisor and/or physician of unusual reactions and/or changes in client’s condition
- Document all appropriate observations and treatments in keeping with Haven Home Health’s policies and procedures
- Participate in case conferences, team meetings, staff meetings and performance improvement activities as assigned
- Provide any skilled nursing service for which appropriately trained which is prescribed under the plan of care
- Record all pertinent observations and treatments and prepares clinical progress notes as per Haven Home Health policies and procedures
- Other duties assigned by supervisor and for which he/she is qualified
- Adheres to all Haven Home Health policies and procedures including but not limited to the HIPAA privacy rule
QUALIFICATIONS:
- Must be a graduate of an accredited license practical/vocational nursing program
- Must be licensed in the state(s) of the Haven Home Health Care office. In the event of working in a state not included in compact licensing, a valid nursing license must be held under said state
- Provide a copy of RN license/renewal, at required intervals and prior to the expiration of previous license expiration date
- At least two (2) years of home care experience with one (1) year supervisory experience

Pay: $95,000.00 - $100,000.00 per year
