The staff L.V.N. is a professional caregiver who provides direct and indirect patient care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of expectations, under the supervision of a registered nurse.
Essential Duties and Responsibilities:

Demonstrates current, comprehensive, professional knowledge and skills in conformance with recognized nursing standards including the Patient Bill of Rights, standards for nursing care and patient care, and the Nurse Practice Act.

Demonstrates knowledge of pharmacology, hospital and department policies.

Participates in the development, interpretation, and implementation, evaluation of nursing Plan of Care, reviewing target dates, and documents progress toward goals, under leadership of RN.

Maintains awareness of current hospital and departmental operational standards which impact position responsibilities. Performs effectively in stressful situations to fulfill position responsibilities.

Demonstrates accountability for ongoing awareness of the status of assigned patients. Document data appropriate to the nature and severity of the illness. Communicates appropriate data to the physician and other patient care team members. Collaborates with other disciplines to plan and coordinate care.

Participates in a rotating call schedule to provide continuous availability of patient services.

Responds promptly to changes in workload, adjusting assignments as necessary to achieve optimal productivity and efficiency.

Anticipates potential patient problems, communicates with RN and performs nursing actions to minimize those problems in a timely manner to ensure patient safety.

Identifies and reports unsafe practices and/or procedures, as related to patient care.

Demonstrates the components of the hospital’s core values of compassion, integrity and innovation.

Assures that supplies and equipment are available for patient care.

Demonstrates ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.

Intervenes as guided by the plan of care to implement nursing actions that promote, maintain or restore physical and mental health, prevent illness and effect rehabilitation.

Document patient responses to interventions with 100% completion rate to ensure completeness of patient care.

Assesses the effectiveness of medication administered.

Assesses the effectiveness of comfort measures (including all PRN medications given) within l hour of intervention.

Follows all Patient Care Services generic performance standards for documentation.

Administer medications as prescribed: IM, IV, Sub-Q, Topical, Orally, Rectally using the 5 rights of medication administration.

Perform other duties as assigned.
Education and Experience:
-
Graduation from accredited school of nursing.
-
Current Texas State licensure.
-
2-3 years experience as an LVN
-
Current BCLS Certification.
-
ACLS certified within six (6) months of hire.
-
PALS certified within six (6) months of hire.
-
Maintains professional growth and development though seminars, workshops, and professional affiliation to keep abreast of the latest trends in the field of expertise.
