The Clinical Supervisor is responsible for the day-to-day running and supervision of Ryan White Medical Case Management services in accordance with all program goals, targets, performance outcomes and medical case management Program policies, procedures, and protocols.
These include but are not limited to: medical case management guidelines of medical care, intense psycho social services, compliance with medical appointments and follow up; treatment education, oversight in the following area: (1) assessment of patients (2) development of an individualized service care plans; (3) referral and follow up on services to help patients obtain needed services; (4) monitoring and follow-up of HIV patients enrolled in primary and specialty services.
The Clinical Supervisor will conduct crisis counseling, psychosocial assessments, document all evaluations, treatment and terminations of patients in a timely manner. Accurately document progress notes; consults with behavior health, HIV and primary care providers as appropriate; Maintain expected levels of productivity; Provide direct delivery of clinical counseling services to patients. Perform routine quality improvement activities to monitor productivity of the medical case managers and health outcomes of patients.

3) Participates in the development of HIV Services grants applications, including work plan development, projection of activity levels, etc.
4) Represents Morris Heights’ at appropriate meetings and events as indicated by the Medical Director.
- Attends all staff, and appropriate committee meetings.
- Participates in trainings and workshops for professional development.
- Experience: LCSW required; At least three years-experience working with HIV positive clients required. At least two years post Master’s clinical experience preferred.
- Interpersonal Skills: Ability to maintain internal contact with physicians and employees, ability to act as a resource and motivate these individuals; ability to communicate effectively on an individual basis and in group settings; ability to coordinate services with medical staff & third-party payers.
- Reasoning Ability: Ability to solve analyze and solve problems; Ability to interpret a variety of instructions furnished in written, oral diagram or schedule form; Ability to define problems, collect data, establish facts and draw valid conclusions; ability to deal with abstract and concrete variables.
- Language Skills: Ability to read and interpret documents such as procedure manuals, performance improvement plans; ability to write reports, correspondents and performance evaluations; bilingual (Spanish) desirable.
- Mathematical Skills: Ability to add, subtract, multiply and divide; ability to compute ratio and percentage; ability to draw and interpret various graphs.

- Certifications/Licenses: Certificate of attendance at New York State AIDS Institute sponsored HIV Trainings including trainings specific to treatment adherence, motivational and substance abuse counseling. LMSW or LCSW registration in New York State Preferred.
