Southern Tier AIDS Program, Inc. is seeking a full time Health Homes Care Manager in our Binghamton office. This position provides confidential outreach, intake, and care management services to persons with qualifying chronic illnesses, as well as Health Homes member tracking tasks.
Salary:

$19.24/hour working 35-hours per week, may vary depending upon experience

Associate’s degree (preferred) in Health, Education, Social Work, Liberal Arts, a related field, and/or two years of experience.

Qualifying experience means verifiable full or part-time care management or customer service experience working with any of the following: persons living in poverty, persons with mental illness, homelessness, chemical dependence, HIV/AIDS, and/or other qualifying chronic conditions (diabetes, asthma, hypertension, etc).

Experience with documenting Medicaid billable services preferred.

Organizational skills, record-keeping, time management and communication skills are essential.

Computer literacy required.
This individual is responsible for meeting the transportation and insurability requirements of this job. A valid driver’s license, clean driving record and reliable transportation is a requirement of this position.
