Youth transitioning to adulthood is difficult to navigate and it is especially tough for youth aging out of the state system. These youth often face insurmountable issues such as unemployment, food insecurity, homelessness, incarceration, and early pregnancy which makes their trajectory in life dim and often these issues are faced alone. The Chase Home is becoming the change agent for youth in these situations. The Chase Home recently contracted with the state of New Hampshire to offer a Level 1/Independent Living Program to youth ages 18-21. The Independent Living Program will work with youth to gain necessary life skills such as education, job placement, driver's license, money management, and mental health stability. The IL Program will wrap around support, services, and resources designed to prepare youth for their future as self-sufficient, confident, and positive contributors to society.
The Independent Living Case Manager provides case management, support, and guidance to the youth in our Independent Living Program.
The Independent Living Casemanager is a Part Time position/25 hours and could become Full Time with 15 hours dedicated to the Residential Program functioning as a Residential Counselor (please see job description of a Residential Counselor)
How You Will Be Making A Difference
Assist the IL Coordinator in managing the IL Program.
Assist with the intake and discharge process and ensure all appropriate paperwork and all data entry is completed and delivered in a timely, efficient manner.
Maintain program(s) census and ensure program(s) is following relevant agency guidelines.
Oversee the services and treatment plans of the youth and their families to ensure they are high quality and are consistent with the goals of the Program. Ensure services are coordinated and integrated. Regularly evaluate services and implement plans, with measurable outcomes, for continual improvement.
Collaborate externally with referral sources to ensure a steady flow of eligible clients to program(s).
Maintain the utilization review process. Ensure quality assurance standards are met with regard to treatment planning, record keeping and documentation.
Participate in completing all assessments, surveys and relevant feedback for the state.
Participate in treatment teams, IEP meetings and administrative team meetings.
Participate in crisis prevention and intervention as needed
Provide program leadership in the absence of the IL Coordinator
Establish, review, and implement program(s) policies and procedures. In consultation with the IL Coordinator, continuously consider new processes and initiatives to improve and/or expand services as appropriate.
Complete follow along services as needed
Promote a positive work environment
Provide rotating on call coverage

Qualifications:
Be at least 21 years old
Prior experience working with youth
HS diploma or equivalency
An Associates degree with at least 12 credits in the field of human services
Bachelor's degree Preferred
