This Licensed Clinical Social Worker (LCSW) will be an integral member of the Adult Complex Care team. This team provides complex care management for defined populations of adult patients within the UCSF health system in a primarily remote model. These populations include the most medically and psycho-socially complex patients, high risk or high utilizing patients, those with poorly controlled chronic conditions, those who belong within a specific ACO contract, or other types of groups that might benefit from more intensive support as an extension of UCSF Primary Care or Specialty Care. The Adult Complex Care clinical team is comprised of Advanced Nurse Specialists (ANS) and Licensed Clinical Social Workers (LCSW). As high-level professionals, the complex clinical team is expected to play a role in program development, refinement, and evaluation. Clinically, this LCSW will be responsible for collaboration with the ANSs in completing a comprehensive intake assessment on each patient followed by development of a complex care management plan. A team of consultants including a physician, pharmacist, and a behavioral health professional supports the complex clinical team. The ANSs and LCSWs will implement the care plans with ongoing care management for their panel of high-risk patients including high level care coordination between primary care providers and specialists, patient education, patient and caregiver support, smoothing transitions between sites of care and facilitating access to appropriate community resources. Motivational Interviewing and other counseling techniques are employed in supporting patients’ efforts to identify and work toward their health care goals with improved self-management of their chronic conditions.
The LCSW provides psychosocial assessment, crisis intervention, brief therapeutic interventions, consultation, education, community linkages and supportive services for identified patients, families, and staff. The LCSW coordinates resources and services in response to patient’s psychosocial needs. The LCSW has advanced knowledge and skills in Clinical Social Work and is distinguished from the MSW by leadership activities within the Care Management team and The Office of Population Health, including participation in advanced program planning, provision of educational program and consultation to the clinical team, service lines, other departments and the community. In addition, the LCSW will serve as a clinical resource to other members of the Clinical Programs in the Office of Population Health, who may be providing interventions such as screening, health coaching and care coordination to identified high-risk cohorts of patients with chronic illnesses.
The LCSW demonstrates the ability to work with any patient regardless of race, age, gender, religious affiliation, cultural beliefs, lifestyle, disease process or treatment plan within an interdisciplinary team. Central to this social worker role is the imperative to forge a partnership with the patient, thoroughly evaluate their needs and barriers to care, and deliver unwavering support to ensure that every aspect of the care coordination is addressed, and access to necessary resources is secured.
This LCSW role is part of a dynamic Care team within the Office of Population Health (OPH) Clinical Programs Care. This position will report to the Manager, Complex Care. The LCSW is a fully remote position with some requirement for travel to San Francisco or the Brisbane Office to meet Divisional priorities.
The incumbent should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $41.79 - $72.17 (Hourly Rate).
