The purpose of this nurse home visitation program is to support clients and their family to achieve the outcomes established by Every Child Succeeds, and HANDS. The goals are to improve pregnancy outcomes, improve childrens health and development and to improve the mothers personal development as described by Every Child Succeeds (ECS)/ Healthy Access Nurturing Development Services (HANDS). The program is focused on low-income first time mothers beginning contact during pregnancy and continuing for the first two years after the child is born.
Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.
- Promotes three aspects of maternal function:

Health related behavior during pregnancy and the early years of the childs life.

The physical, social, and emotional care parents provide their children.

Parents own life course development.

Links families with needed health and human service.

Coordinates community services with families. In addition to primary health care providers, linkage with Community Based Services, local health departments, hospitals, mental health services, and housing are required to assure provision of the full range of health services needed by enrolled children and families.

Provides active promotion of positive health related behaviors and specific qualities of infant care giving instead of focusing solely on social support.

Focuses on families in greater need of services, including familiarity with low birth weight and pre-term infants, children with chronic illness and disabilities, low income unmarried teenagers, parents with low IQ, and families with a history of substance abuse.

Provides measures to reduce family stress by assisting in improving their social and physical environments.

Maintains required case load for board hours.

Involves the family members and friends in the pregnancy, birth, and early care

of the child.

Performs other duties as assigned.
Education, Credentials, Licenses:
-
Associate's Degree in nursing or diploma from an accredited nursing school.

Licensed to practice in the state where work is being performed.

Meets contact hour requirements for licensure, including all state-required courses.
Specialized Knowledge:
Previous work experience in Maternal Child Community Health Nursing. Attend training program from Prevention Research Center for Family and Child Health.
Kind and Length of Experience:
Previous work experience in Maternal Child Community Health or Mental Health Nursing
