Title: HIV and Gender Affirming Care Nurse Care Manager (RN)
Work Location: 8 E 3rd Street and 200 Varick Street, New York NY
Hours: 8-4 remote/hybrid schedule.
Reports to: Chief Medical Officer
Salary: 115K-125K
Position Overview:
PRI seeks a registered nurse for a hybrid clinical and administrative role. Clinically the RN will support patients and clinic staff in two main areas: HIV care management, prevention, testing, and counseling; and gender affirming care care coordination and clinical support. Administratively, the role collaborates on quality assurance and quality improvement efforts for the HRSA funded HIV program activities, data analysis, and reporting.
Essential Duties & Responsibilities:
The essential duties of the HIV and Gender Affirming Care Nurse Care Manager include but are not limited to the following activities:
Clinical
- Participates in HIV clinical meetings and meets with patients at the HIV clinic
- Provides counseling along the HIV care continuum including testing, Pre-exposure prophylaxis (or PrEP), and Post-exposure prophylaxis (PEP)
- Works with medical case manager to ensure all new patients with HIV are connected to care in a timely manner
- Supports patients living with HIV with medication adherence and counseling to achieve and maintain viral suppression
- Collaborates with facility behavioral health programs on patient needs for HIV prevention and management
- Coordinates care for patients receiving gender affirming care, including assisting with referrals, providing injections, and connecting patients to providers
- Facilitates referrals to community-based psychiatric, and other community-based resources to address social determinants of health
- Serves as point of contact for patients requiring assistance with clinical needs; triages needs to other other team members as indicated
- Reconcile external documentation for patient records as needed to maintain ongoing quality improvement efforts.
Administrative:
- Provide supervision to Community Health Worker and Peer for work on improving viral suppression rates
- Take the lead on tracking HIV quality metrics and reporting to the team and external agencies
- Collaborate with HIV team and Quality Improvement team on quality improvement projects related to HIV and gender affirming care
- Support Assistant Director for Clinical Quality Improvement with HIV related data for internal and external reporting requirements
- Participate in state and federal Ryan White Program HIV trainings, conferences, and meetings.
- Participate in HIV related educational events with patients including quarterly Client Advisory Board.
- Provide agency training on HIV care and Gender Affirming Care
- Maintains documentation in compliance with agency policies and procedures.
Note: This role will travel as needed to support HIV services and trainings at all PRI clinic sites.
Qualifications:
- New York State Licensed Registered Nurse
- 3-5 years of clinical experience
- Experience in care management preferred
- Experience working with people experiencing homelessness preferred
- Must have excellent oral, writing and listening skills
- The ability to work independently, prioritize tasks, and create and coordinate schedules
- Must have the ability to interface with clients as well as all levels of staff
- Must have knowledge of Microsoft Office, especially Word and Excel
- Knowledge of eClinicalWorks is a plus
