Provides nursing care for assigned residents according to established standards and practices relating to basic nursing, restorative and personal care services where quality assurance, resident safety and infection control are inherent responsibilities of the job. Under the supervision of the community executive director the LPN will provide support and leadership to the community caregivers, provide superior resident care and build meaningful relationships with the seniors and their families.
ESSENTIAL DUTIES AND RESPONSIBILITIES: Other duties may be assigned
1.Communicates resident health problems and situations to Director of Health Care (DHC).
2.Ensures that staff observes residents for significant changes, adverse reactions, emesis, drainage, bleeding and communicable diseases. Ensure that resident's needs are met or fulfilled.
3.Communicates all changes with residents to the Director of Health Care (DHC) and responsible party at the direction of the DHC.
4.Prepares, administers, and charts the administration of prescribed medications; documents effectiveness and adverse reactions.
5.Process daily physician orders as directed to do so by DHC.
6.Reviews all MARs on a daily basis.
7.Ensures resident’s medications supply is maintained on a daily basis, i.e. re-ordering refills and/or new orders.
8.Ensures and performs personal comfort activities such as giving bed bath or shower or assisting with bath or shower. Observes skin and pressure areas ;assists with personal hygiene services such as oral hygiene, hair care, nail care, shaving, dentures, eye wear and makeup, etc.
9.Prepares for and participates in care conferences.
10. Prepares and monitors residents for meals, and between-meal nourishment; assists in positioning; sets up table; feeds residents if necessary.
11.Responds to nurse call and paging systems; monitors residents to ensure the resident remains clean, dry and comfortable by changing clothes, gowns, linens, dressing as necessary; provides clean water and refreshments, assists in keeping the room clean and neat by straightening and replenishing person supplies.
12.Monitors resident safety on a continuing basis, seeks assistance in emergencies, assists family members in times of stress or crisis, perform established notification procedures during emergency situation.
13.Takes and records at predetermined intervals vital signs such as temperature, pulse, respiration, weight and blood pressure, measures intake. Collects sputum, urine, and stool, discharge drainage and other specimens as ordered.
14.Performs periodic cleaning duties, maintains orderliness of the nurse’s station; collects and disposes of contents in trash containers.
15.Works cooperatively with members of the care team, maintains effective interdepartmental and interpersonal relations residents, staff and visitors, avoid unnecessary noise.
16.Prepares incident reports and takes appropriate action. Report incident to DHC and responsible party at the direction of the DHC. Completes incident report before the end of the shift.
17.Performs other administrative duties such answering phones and other related duties.
18.Participates in all new resident admissions.
19.Sets priorities in implementing resident care
20. Prepares and monitors staff scheduling for the designated shift
21.Monitors all care team members on shift to ensure the highest quality care is provided at all times.
22.Participates in training of new staff
23.Communicates with Director regarding staffing concerns and/or problems
24.Participates in corrective action of team members when necessary.
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.
Knowledge: General knowledge of professional basic nursing services, restoratives and personal care services, general knowledge of resident mental health and social needs and residents rights.
Skill: To apply professional basic nursing, restorative and personal care services as evidenced by a current license to practice as a LPN.
Ability: To walk and/or stand most of the day, to work in reaching or strained positions most of the day, to lift carry or handle light weight materials ( 1- 25 lbs or resident-equivalent lbs) half day, medium weight (26-60 lbs or resident-equivalent lbs)materials frequently, and heavy weight (over 60 lbs or resident-equivalent) materials occasionally, intermittent sustained muscular exertion.
To follow verbal and written directions, to supervise effectively assigned staff to work effectively with others in a responsible and cooperative manner, to assist residents with issues such as retaining optimum independence and death and dying, to maintain personal and dress uniform cleanliness
Required Educations: Completion of an approved Licensed Practical Nurse Program, or the equivalent, approved by the state Board of Nursing Examiners and licensed to practice as a practical nurse and has a copy of license on file in the personnel record. Must be first aid and CPR certified.
