Job Title: Public Health Nurse (FUSE Program) - RN or LPN
Starting Hourly Rate: $25.7728
Position Status: Full-Time
Work Schedule: Monday through Friday, 8:00 to 4:30 p.m.
Position Summary
McLean County has prioritized improving the lives of those experiencing behavioral health concerns as they intersect with the homeless and/or criminal justice systems. With this goal in mind, a Frequent Users System Engagement (FUSE) program has been created to link individuals with housing and offer intensive wrap-around behavioral health supportive services. This position will be a part of the program’s collaborative staffing model.
Under general supervision of the FUSE Program Manager, provides a variety of professional nursing services to patients in both an office/clinic environment and in the community. Work requires careful monitoring of the person’s health and behavior, performing professional nursing services, and reporting unusual or different conditions to a physician, a nursing supervisor, or an administrative supervisor. Work is performed according to professional nurse training, OSHA and professional standards, public health, and County policies and procedures. Services performed require employees to communicate with residents in a friendly and courteous manner.

Essential Duties and Responsibilities
- Position requires on-call phone coverage (7 days every 3-4 weeks) shared with other team members.
- Observes and assesses patients' physical and mental conditions; maintains medical and other records of a patient’s conditions.
- Provides nursing care according to needs, provider direction and accepted policies; arranges for patient transportation to a hospital or other medical facility; maintains patient medical records.
- Organizes, maintains, and transports client medications in a secure manner, including controlled substances.
- Provides injections prescribed to clients by the FUSE program provider
- Communicates with others to convey and request information.
- Communicates with patients in a pleasant, friendly, and courteous manner.
- Maintains documentation necessary to facilitate compliance with professional standards, policies, and regulations
- Provides case management services to clients when team members are unavailable.
- Responds to telephone crisis calls as assigned and triages appropriately.
- Transports clients to medical appointments, or other appointments as assigned.
- Performs related work as required.
Knowledge, Skills and Abilities
- Knowledge of modern theory and practices in professional nursing.
- Knowledge of human anatomy as it relates to treatments and therapies.
- Knowledge of the more common physical illnesses, obvious symptoms, and appropriate medical terminology.
- Knowledge of trauma and how it affects human interaction and behaviors.
- Knowledge of behavioral health diagnoses and medications commonly prescribed.
- Knowledge of the methods used to calm excited patients or relatives.
- Knowledge of effective communication techniques designed to make people feel valued.
- Ability to monitor a patient’s physical and mental condition and take appropriate action.
- Ability to meet professional standards, as required by the position.
- Ability to respond to each patient with empathy and respect while implementing prescribed treatment.
- Ability to accurately read, interpret and clarify providers' orders and provide correct medications to patients as directed and needed.
- Ability to communicate in a friendly, pleasant, and courteous manner.
- Ability to read, understand, and apply County policies.
- Ability to establish and maintain effective working relationships with superiors, other employees, doctors, patients, and relatives.
Required Qualifications
- Graduation from an approved school of nursing and licensed as a Licensed Practical Nurse or Registered Nurse in the State of Illinois.
- LPN applicants should plan to gain RN licensure by 2 years after hire date
Relevant or Preferred Qualifications
- Basic Life Support Certification, as required by the position.
- Mental Health First Aid
- Additional Professional Certification related to Psychiatric-Mental Health, Substance Use/Addictions, Ambulatory Nursing, and/or Home Health
Physical Attributes/Demands
- This position requires activities such as sitting, walking, standing for long periods of time, bending, stooping and lifting light to medium objects.
Work Environment
The work is performed in community, in the living arrangements of patients, and in a health care facility and may involves exposure to body fluid, cleaning chemicals, air borne diseases, and angry/combative patients. Latex gloves, gowns, and facial masks are worn. Back braces or gait belts may be worn, as needed.
