Essential Qualifications:
- A Bachelor’s Degree in Nursing or completion of an accredited LVN program.
- At least two years of experience in Quality, Health Planning, Case Management, or Population Health.
- Familiarity with electronic health records and a history of working on QI projects.
- Current LVN License and BLS Certification.
Preferred Qualifications:
- Advanced degrees in Nursing, Public Health, or related fields.
- Extensive experience in quality, planning, case management, or population health.
- Proficiency in languages other than English, especially Spanish, and experience with diverse populations.
- Familiarity with healthcare platforms like Allscripts, FollowMyHealth, and others.
