Summary/Objective
The RN Case Manager is a registered professional nurse who ensures quality of patient/client care in the home. The primary function is the day-to-day operations of assigned agency area and clinical services to ensure quality care and efficient operations. Coordinates with the parent office, the interdisciplinary team, patient/client, their family, and the referring agency. Oversees clinical issues at the branch level and directs staff regarding Agency policies and procedures. Maximizes human, financial and equipment resources.

Essential Functions*
- Nursing skills as defined as generally accepted standards of practice.
- Evaluation, management, and coordinates services of assigned agency location and assumes responsibility for continuity, quality, and safety of services delivered.
- Identifies problem areas and reports to the Administrator and the Director of Nursing/Supervising Nurse at the parent office.
- Promotes the philosophy, mission, and administrative policies of the Agency, in an effort to ensure quality of patient/client care.
- Employs, recruits, manages, and directs qualified personnel to carry out the policies and procedures of the Agency.
- Ensures staff education and evaluations.
- Demonstrates commitment, professional growth, and competency.
- Promotes and monitors compliance with fiscal intermediary and/or other third-party payors, federal and state regulations, through education, coaching, the QI program and other assistance as necessary.
- Monitors operational systems for efficiency and compliance with regulations and identifies problem areas to Administrator and Director of Nursing/Supervising Nurse.
- Meets mandatory continuing education requirements of the Agency and licensing board.
- Provides communication to the parent office regarding needs of assigned area to include personnel needs, financial needs, professional practices, quality issues and planning.
- Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
- Preferred/Required Education and Experience*
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Licensure: Current Texas State license as a Registered Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC), current Texas driver’s license.
- Minimum one year's experience as a registered nurse and one year's experience in home health and pediatrics strongly preferred. Management or supervisory experience of one year strongly preferred.
- Required proof of current BLS for Healthcare Provider CPR.
- Ability to complete skills check off upon hire.
- Willingness and ability to drive to patient’s homes and office for patient care/supervisory responsibilities.

- Performs duties in an office and patient home environment during Agency operating hours and to make patient/client visits in patients’/clients’ homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; must have the ability to work a flexible schedule, and to travel required based on patient location/need (maybe extensive at times across state); some exposure to unpleasant weather; on call rotation required; driving required to assigned area.
- Prolonged sitting is required. Requires ability to always handle stressful situations in a calm and courteous manner.
- Requires working under some stressful conditions to meet deadlines and agency needs.
- If required to make patient/client visits, prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients/clients. Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition.
- Position Type/Expected Hours of Work*
- This is a full-time salaried exempt position. Typical days and hours of work are Monday through Friday, 8:00 a.m. to 5 p.m. with On-Call rotation.
