4 2 Restore, LLC is seeking a compassionate and dedicated Direct Care Worker to join our team. As a Direct Care Worker, you will play a crucial role in providing support and assistance to individuals with mental health challenges in their homes and communities. The services provided are individualized and based on detailed client assessments. As a Direct Care Worker, you will be able to continue your career path in the Social and Human Services field.
Duties:
- Work closely with individuals with mental health challenges to provide personalized care and support
- Assist with activities of daily living, teach independence and self-advocacy
- Ensure a safe and clean environment for individuals under your care
- Develop and implement Treatment Plan and follow protocols to meet the specific needs of each individual
- Help clients resolve problems, identify resources, and make effective use of resources
- Provide companionship and emotional support to promote overall well-being
- Provide a minimum of 6 or 4 hours of direct service, per client, per month
- Prepare and maintain written case records, reports, notes, and other documentation in ICNotes
- Adhere to HIPAA regulations and maintain confidentiality at all times
- Perform administrative tasks as required
- Participate in and complete all required training
- Other duties as assigned

Requirements:
- Associate's Degree or Bachelor's Degree in a Health-Related field
- 2-yrs. Work Experience in Mental Health
- 2-yrs. experience ICANotes note writing skills
- Submit to and pass a Pre-Employment drug test
- Pass State required background screenings
- Completed CPR/First Aid training
- Possess a valid Driver’s License in good standing
- Reliable transportation and auto insurance that complies with the minimum state requirements
- Ability to travel locally
- Attend mandatory weekly & monthly meetings as required by the agency

ICANotes - 2-yrs (Required)
Microsoft Office 365: 2-yrs. (Required)
License/Certification:
Maryland Driver's License (Required)

$18/hr -$20/hr - based on experience
