Job Title: Registered Nurse (Mandarin/Cantonese Speaking)
Department/Program: Nursing
Under the direct supervision of ADHC/CBAS Program Director, the Registered Nurse is responsible for the provision of ADHC/CBAS nursing services to participants receiving medical care, including assessment, care plan development and monitoring of each participant, per Title 22 regulations. Participates in multi-disciplinary team meetings and provides consultation to therapists, families, and participants.
Basic Responsibilities for Job Title:
Full-time position or Part-time position
1. Provides excellent customer service during the center's hours of service.
2. Assesses each participant to determine needs for personal care and assistance in activities of daily living such as bathing, grooming, toileting and eating. Include these needs in the plan of care and supervise the provision of these services. All participants must be reassessed at least every six months.
3. Specifies short-term nursing goals in each individualized plan of care/IPC. Provides active role as member of the multi-disciplinary team to assess the nursing needs of the participant.
4. Develops the nursing component in the plan of care for each participant capable of benefitting from a continuous training/program.
5. Documents (flow sheets) all skilled nursing care provided on the day the service is given and includes quarterly signed and dated progress, per Title 22. notes in each participant record.
6. Provides health education and counseling to the participant and family when indicated by the plan of care.
7. Aids the participant in obtaining medical services from providers other than adult day health staff.
8. Monitors, administers, and records prescribed medication as per physician's orders.
9. Provides training in self-administration of medications to participants.
10. Provides liaison to the participant= personal physician.
11. Provides supportive and restorative nursing as indicated by the treatment plan.
12. Provides observation and monitoring of health status.
13. The nurse will contact 911 and request an ambulance or paramedic unit depending on the condition of the participant.
14. Supervises and signs the Physical/Occupational Therapy aides and Maintenance How Sheets, respectively, in the absence of one or both therapists.
15. May be assigned other duties by the Program Director.
Job Types: Full-time, Per diem
Pay: $43.00 - $48.00 per hour
