The pay range for this job is on a 14-step scale starting at $4,952.00 per month. Most candidates start at the first step.

Our Agency is part of the national network of Area Agencies on Aging, a public service agency dedicated to helping persons aged 60 and over, adults with disabilities and their families to access needed community services in Clark, Cowlitz, Wahkiakum, Klickitat, and Skamania Counties. This position serves clients in Eastern Klickitat County.
The Care Coordinator provides support for designated clients/beneficiaries which includes coordinating an array of services designed to improve the health of high needs, high risk clients/beneficiaries. Primary responsibilities include assessment, care planning and monitoring of client status, and implementation and coordination of services. Provides support to clients/beneficiaries for effective care transitions, improved self-management skills and enhanced client/beneficiary-provider communication. Also facilitates interdisciplinary consultation, collaboration and care continuity across care settings.
Requirements include: Bachelor's degree in social work, human services or related field with two years of paid on the job social service experience. Experience working on cross disciplinary, cross-organizational teams and experience meeting and working with people in homes and other medical and community settings required.
Must possess reliable transportation, a current driver's license and minimum state required vehicle insurance. A background check will be administered.
While performing the duties of this position, the employee is regularly required to talk, hear, stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls, and reach with hands or arms. The employee occasionally lifts or moves up to 50 pounds. Specific vision abilities required by this job include close and moderate-distance vision and the ability to adjust focus. To perform this job successfully, an individual must be able to perform each essential job duty satisfactorily.
