Who We Are:
Elliot Health System’s Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) provide individual and group-based treatment of co-occurring substance use and mental health disorders. PHP and IOP are for individuals who need care that is more intensive than individual outpatient programs but not as intensive as residential treatment. Depending on their role, staff can provide case coordination, assist participants in accessing community-based services, provide medication management, evaluate participants for medical issues, and more. The teams coordinate closely with primary care providers on a biweekly basis to ensure that all aspects of patients’ wellbeing and health are addressed appropriately.
About the Job:
The Drug Court Case Manager is responsible for providing casework services to clients enrolled in the Drug Court program. The Drug Court Case Manager will work closely with the Drug Court Coordinator and the Drug Court teams.
What You’ll Do:
Testify in court regarding offender compliance with court order, as needed.
Screen offenders at the jail for entry into program as indicated.
Maintain a case load of a minimum of 25 clients.
Prepare for client discharge by reviewing and amplifying discharge plans. Coordinate discharge and post-discharge requirements.
Establish treatment program by setting schedule and routines; conduct services provided, arranging resources including transportation and escort.
Manage application process and ensure that all other required documentation has been received.
Attend treatment team meetings (in both locations) for case consultation and decision making.
Connect clients with resources, assisting client in pursuing the best possible options. Assist clients with job training and employment searches; work with area employers to help place clients in appropriate jobs.
Assist clients with regards to public assistance programs, financial education, and housing procurement.
Assist clients with accessing health and mental health care and support groups.
Who You Are:
Education: Bachelor's Degree from an accredited four-year college or university with a major in criminal justice, social services, behavioral health, psychology or a closely related field required.
Licensure/certification: Annual twelve (12) hours of continuing education and training in substance abuse and criminal justice issues required.
Experience: One (1) to three (3) years of experience working in a behavioral health field required. If in recovery, provide evidence of personal stability, as well as adhering to all organizational policies as it relates to recovery required.
