JOB SUMMARY: Under the direct supervision of the Assistant Director, the Prevention Navigator/Testing Specialist is responsible for outreach, recruitment, and providing IDI Sister-to-Sister prevention education and STI/HCV testing services, with a focus on adult women identified as high-risk for STI/HCV/HIV, substance use, mental illness. Under the direct supervision of the Program Manager, The Prevention Navigator/Testing Specialist works along with the Health Educator and will conduct linkage and navigation services and Prep support for qualifying clients.
**This role has a variable schedule. General schedule 11 AM to 7PM preferred. **
EDUCATION:
Minimum High School Diploma or equivalent required
Minimum Bachelor's Degree strongly preferred.
WORK EXPERIENCES:
· Minimum: Three (3) years of case management experience
· Minimum: Two (2) years of experience conducting HIV /Hep C testing and delivering phlebotomy services for STI testing.
· Promoting and recruiting for program services, working and providing services to and for Women of Color ages 18-45 with linkages to care such as Prep, entitlements, and HIV/STI/Hep C testing.
· Strong documentation skills and case management. Knowledge of referrals and provider networks.
Certification: HIV/HCV testing, Phlebotomy, or Medical Assistant
Licensure: NYS Driver’s License in good standing and two years of driving experience strongly preferred
DUTIES AND RESPONSIBILITIES:
· Essential Duties: Conduct all services and satisfy all deliverables as stipulated in the program contract.
· Interventions Delivered to Individuals (IDI): Conduct Sister to Sister as a one-on-one, woman-to-woman, brief intervention. Offering knowledge and skills needed to reduce the risk of HIV. The intervention should seek to empower women of color to take control of their health by choosing pre-exposure prophylaxis (PrEP) and/or using condoms consistently to prevent HIV.
· Outreach/Marketing: Create linkages and partnerships with other providers to recruit the target population. Attending outreach events, health fairs, tabling, etc. and recruiting the population of focus for enrollment into the program. Attend weekly outreach and social media meetings. Outreach events may include some evening and weekend scheduling.
· Linkage, Retention & Adherence Services: Conduct Behavioral Assessment of clients’ health risk and HIV status to determine a client’s risk of acquisition of HIV and STD/HCV. Develop an Action Plan to address needs identified in Behavioral Assessments. Provide linkage and navigation services (4 per client) such as housing, education, employment, 340B, benefits and entitlements. Provide PrEP Linkages to all high-risk/at risk HIV – women or couples Make referrals and linkage navigation for ancillary needs (mental health, substance use, case management, and other services) Provide escorts, case conference, follow up calls and assistance to assure a successful linkage.
· Screening /Testing Services: Conduct at min 150 STI Testing and counseling and document these services within 24 hours or concurrently. Obtain informed consent on NYS consent forms. Ensure that copies are placed in the clients’ records. Provides accurate records/documentation and statistics that are required by the NYS Department of Health.
Additional Duties:
· Actively enroll qualified clients into the 340B program
· Attend outreach, training, and conferences as they relate to your program and target audience.
· Participate in assigned agency committee meetings
· Document and follow all procedures in accordance with DOH and specimen handling
· Maintain updated and accurate individual records for all clients in the eCR
· Submit all AIRS forms within 24 hours of providing services capturing all work done with the client
· Complete chart closing within BAC and AI policies
· Attend supervision, team meetings, and department meetings
· Create linkages/MOUs and establish relationships with organizations/programs where you may refer clients.
· Other duties as assigned as needed based on program needs.
KNOWLEDGE, SKILLS, AND ABILITIES:
· Certified Phlebotomist, HIV/HCV Tester or Medical Assistant (Required)
· Previous work experience with YMSM, high risk and/or HIV+ populations, a plus
· Previous experience delivering curriculum evidenced-based facilitation, a plus
· Providing clients with testing results and making referrals for treatment as needed
· Effective communication and documentation skills (written and electronic).
· Ability to work with people of various racial/ethnic backgrounds
· Computer literacy is required.
· Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodations.
· Must be willing to work weekends, nights, and long/flexible hours as required by the position
· Must be willing to conduct outreach/travel 75% of the time.
