Stafkings Healthcare Systems is currently recruiting homecare RN’s to perform assessments, admissions and aide supervisions. We have full-time and part-time opportunities to provide home care case management in Binghamton with periodic travel to the surrounding counties.
Experience and Qualifications: NYS RN license from an accredited school of nursing and current NYS registration. One year of nursing experience in home care and physical assessment or an equivalent combination of nursing experience in a hospital, physician’s office, long term care facility or other medical setting. Homecare and supervisory experience a plus. Maintains a current NYS driver’s license and auto insurance. CPR certified.
Position Summary: The RN provides professional nursing care visits to patients primarily in the patient’s homes. Admits patients and coordinates their home care course of treatment with members of the patient’s care team. Confers and collaborates with physicians in the implementation of the home care treatment plan. Independently provides and documents patient visits. Oversees and supervises aide field staff and re-educates policy and procedures as needed.
- Performs home visits to include: initial admission assessments, supervisory and PRN re-assessments.
- Develops and implements individualized patient plan of care and Instructs patients and caregivers according to the plan of care.
- Documents and submits client visits and enters patient information/updates/changes, (i.e.,: clinical notes, changes in diet, allergies, DME, etc.) for patient’s care within established time parameters (48 hours).
- Supervises home health aides, personal care aides, companions and licensed practical nurses in the provision of patient care. Performs competency evaluation on the aides prior to their provision of client care and annually or more often as needed. Participates in field staff’s 90 day and annual performance evaluations.
- Reviews aide documentation on a weekly basis, educates/re-educates staff if needed on proper documentation.
- Reports progress and changes in patient’s condition to physicians, family and referral sources on an ongoing basis.
- Obtains physician orders as patient care needs change and as needed on an ongoing basis.
- Revises the plan of care based upon assessment findings.
- Administers and reads PPD’s
