The DPCS position is the utmost nursing leadership role within the agency. The DPCS works directly with administration, and shares responsibility in all matters relevant to maintaining smooth day to day operations.

DPCS Responsibilities:
- Coordinate and supervise the delivery of hospice services, activities and patient care provided by an interdisciplinary staff/team in the home setting; formulates and monitors quality and performance criteria, policies procedures and service standards. Staff may include RNs, LVNs, and Aides.
- Establish, implement, and maintain patient care and service standards to meet member service needs and expectations.
- Monitor and facilitate workflow, maximizing utilization of resources to provide the highest quality of care.
- Manage staff, provide clinical supervision and implement, deliver and evaluate/ improve assigned programs and services.
- Ensure compliance in meeting all regulations and standards related to hospice, Medicare, TJC, Title 22, and other federal, state, and local requirements.
- Ensure accuracy and completeness of patient records.
- Identify service delivery/quality issues and implement corrective actions/plans for improvement.
- Determine the appropriate staffing requirements and develop processes to interview, hire, train and maintain the competency of all department staff.
- Assess professional development needs of staff and plans for training/continuing education of professional staff.
- Provide clinical leadership and support to the agency team.
- Act as patient advocate resolving patient care issues. Interpret and explain the program benefits to members and caregivers.
- May provide liaison, monitoring, consultative expertise, orientation and continuing education to contract agencies.
- Participate in special projects; attend and/or conduct meetings, committees with the objective of increasing agency market share and improving quality of patient care
- Conduct a quality management/improvement/assurance program to ensure compliance with standards for hospice services/programs and external regulatory agency requirements.
- Evaluate clinical outcomes and performance of programs/services and make appropriate recommendations for new modalities/programs or modification/improvement of existing programs.
- Ensure practices, services and operating procedures comply with administrative, legal and regulatory requirements of the Health Plan and governmental and accrediting agencies.
- Must be able to work in a Labor/Management Partnership environment, and perform other duties as necessary to ensure the safety and well being of APF Hospice patients.
Qualifications
- Management and supervising experience
- Proficiency in EMR systems and medical terminology
- Strong background in nurse management and physiology knowledge
- Knowledge of HIPAA regulations and medical records management
- Experience in shift management and overseeing nursing staff
- Premium skills in critical care, hospice care, and home health
- Familiarity with Medicare regulations and resident's rights in healthcare settings
