Resident Care Director/ Case Manager

The Case Manager/RSD is responsible for the admission of all new residents to the assigned communities. The CM/RSD is to create a customer service experience for seniors and their families that is in accordance with Willow ridge Senior Living values.
The CM/RSD is to schedule/link community resources and benefits for residents and their families, to help maintain a happy, healthy living environment.
Essential Functions:
- Initial and periodic evaluation, at least once every 12 months, of the need for a resident and of the capability of the community program to meet those needs.
- Orient all new residents and family to their daily routine.
- Assist each resident to adjust to life in the community.
- Assist each resident to maintain family and community ties and develop new ones
- Encourage and promote resident participation in community activities.
- Establish linkages with and arranging for services from public and private sources of income, health, mental health, and social services.
- Assist residents in making applications for and maintaining income entitlements and public benefits.
- Assist the resident in obtaining and maintaining a primary physician or source of medical care of choice, who is responsible for the overall management of the individual’s health and mental health needs
- Assisting the resident in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the residents health or mental health, including
- Oversee all aspects of the Medication program.
- Assist residents in receiving medications and dental health.
- Provide information and referral.
- Assist residents in need of alternative living arrangements to make and execute sound discharge or transfer plans.
- Assist in the establishment and operation of a system to enable residents to participate in planning for change or improvement in the facility operations and programs and to present grievances and recommendations.
- Provide such case management services as are necessary to support the resident in maintaining independence of function and personal choice.
- Manage and Review Incident report process and case manage each incident.
- Implement and manage the schedule for the Wellness dept.
- Quality Assurance for the Wellness Program
- Assist with the hiring, discipline, and training for all Wellness staff.
- All other duties as assigned.
Job Type: Full-time
Pay: $64,000.00 per year
