Connected Health is seeking a board-certified primary care physician (Internal Medicine or Family Practice) to become a valued member of our innovative and patient-focused team.

Primary Responsibilities:
- Provide exceptional, patient-centered, comprehensive primary care services within a concierge model
- Develop and implement appropriate treatment plans for patients, maintaining a proactive approach to care and ensuring timely follow-up communication
- Perform comprehensive annual physical exams, sick visits, wellness visits, and conduct some home visits and virtual consultations in addition to in-office appointments
- Participate in after-hours patient care when required
- Coordinate care with specialists and collaborate respectfully with outside physicians and other members of multidisciplinary patient care teams
- Focus on creating value for patients and enhancing the member experience
Requirements and Qualifications:
- Board certification in Internal Medicine, Internal Medicine/Pediatrics, or Family Practice
- State of Pennsylvania Medical License (required)
- Experience in primary care or internal medicine (preferred)
- Proven work ethic
- Demonstrated success in building deep, respectful, and long-term relationships with patients
- Excellent medical and diagnostic expertise with the ability to build strong rapport with patients
- Commitment to patient advocacy and leading-edge, evidence-based medicine
- Superior verbal and written communication skills, and outstanding bedside manner
