JOB SUMMARY:
The Staff Nurse, LPN participates in the coordination, planning, implementation and evaluation of care provided by the Patient Care teams under the direction of the provider team leader but within the objectives, standards and policies of the center and within the parameters of his/her preparation. Participates in care team quality improvement activities including care team performance on required clinical, operational and financial measures. Ensures the health, comfort and safety of patients.
ESSENTIAL FUNCTIONS:
- Participates in Care team huddles- initiates pre-visit planning workflow to ensure care completion prior to visit whenever possible.
- Exhibits excellent customer service at all times. Maintains a professional appearance and attitude and displays to internal and external customers at all times
- Monitors and stays aware of the customers wait time at all times.
- Perform Patient assessment for non-appointment patients (walk-ins) and all phone calls.
- Prioritize walk-ins according to the urgency of the patient’s condition.
- Assess vital signs (temperature, pulse, respiration, blood pressure, weight, height, etc).
- Place patients in examination rooms with proper drapes and privacy.
- Prepare patients for exam procedures.
- Keep exam rooms clean, organized and stocked.
- Comprehensive assessment including documentation of symptoms, chief complaints, medication reconciliation and allergy history.
- Initiate standing orders related to established guidelines adopted by FPHC.
- Administer medications as authorized and prescribed by physician.
- Documents properly all treatments, medications, lab results, phone messages, referrals, etc.
- Assist physician with treatments, examinations, injections, x-rays, EKG’s, holter monitors, assessments, etc.
- Assist and respond to emergencies.
- Monitor supply levels and reorder when needed.
- Ensures coordination and tracking of all labs, referrals and diagnostic tests as required.
- Maintains crash cart, O 2 tanks, narcotic box, refrigerator, medical supply room, medication closet and IV cart. Responsible for completing and reporting findings on log sheet to supervisor at the end of the reporting period.
- Involving the patients in activities to improve their health (patient engagement). Use skills for patient assessment and implements patient teaching/education/ self management.
- Participates in PCMH Care team quality improvement activities including care team performance on required clinical, operational and financial measures.
- Performs venipuncture and basic lab work as needed.
- Performs other duties as assigned.
- ADDITIONAL RESPONSIBILITIES:
- Maintains current Alabama Nursing License and CPR certification. Remain abreast of current employee health practices and guidelines through continuing education classes, journals and professional organizations.
- Attends continuing education related to infection control practices, monitoring and surveillance of infectious disease processes within an organization.
EDUCATION AND EXPERIENCE:
Diploma in Nursing, graduate from an accredited school of nursing. Current license to work in the State of Alabama. One year clinical work experience preferred.
