Position: Program Manager
Status: Exempt (1.0 FTE)
Dates: One-year position (July 1, 2024 to July 30, 2025)
Salary: $84,250/yr.

CHP is seeking a Program Coordinator III (PCIII) position that will work in our Population Health team to manage the day-to-day activities of specific quality improvement and managed care projects. The PCIII will play a vital role in the development and implementation of its Kaiser Improving Quality & Population Health Management grant, value-based care activities, managed care communication, and technical assistance with our CHC members. This position reports directly to the Director of Population Health and is based in San Jose, California.
Under the direction of the Director of Population Health, the PCIII will manage grant contracts, provide technical assistance and advocacy to member CHCs, build relationships with key stakeholders, and support the improvement of clinical outcomes. The position requires an organized and motivated professional with comfort and experience facilitating group meetings and/or training, knowledge of managed care, population health, and quality improvement principles, and the ability to work in a fast-paced environment.
Qualifications:
- Master’s degree in Public Health, Public Administration, or a related field; or a Bachelor’s Degree in the previously stated fields with a minimum of 3 years of experience working in healthcare, Medicaid/Medi-Cal managed care, process improvement, or payer experience
- Strong public speaking, presentation, training, and facilitation skills
- Experience with grant management and reporting
- Strong written and verbal communication skills
- Outstanding organizational skills
- Ability to operate all applicable computer software (Google Suite, Word, Excel, and PowerPoint)
- Knowledge and experience of Federally Qualified Health Center operations, billing and reimbursement a plus
- Experience with electronic medical record systems (e.g., Epic, NextGen) a plus
Key Responsibilities
- Execute the day-to-day planning and implementation of assigned population health initiatives, and coordination of EHR optimization and payer strategy activities
- Mange project schedules and ensure adherence to established project timelines, contracts, evaluation, and reporting
- Provide training and technical assistance on QI, population health, and managed care
- Contribute to identifying and sharing evidence-based practices to improve compliance and performance of HEDIS metrics and performance in value-based performance programs
- Assist in the development of payer strategies and committees that support the improvement of member assignments, credentialing, provider paneling, and data exchange
- Participate in and attend CHP staff meetings, team meetings, and stakeholder meetings
- Keep informed of local, state, and national health care quality trends, particularly as they relate to FQHC’s, Medi-Cal Managed Care and the uninsured
- Other responsibilities are assigned to meet project and agency mission, goals, and objectives
Job Requirements
- Valid California driver’s license, access to a vehicle, and proof of liability insurance
- TB clearance & willingness to follow COVID-19 safety procedures per Santa Clara County’s Public Health Officer’s Orders
- Successfully pass a criminal background check
- Ability to attend local meetings in-person
- Ability to work flexible hours, including weekends, and evenings
- Ability to think critically, evaluate quantitative data, and draw valid conclusions
- Ability to adapt to changes, maintain focus on responsibilities, and manage competing priorities
