This position is responsible for assisting individuals with a designated behavioral health diagnosis or physical health condition in addressing their healthcare. The Care Coordinator will oversee and coordinate access to all of the services a member requires to optimize their health. This is a KPERS covered position.
Job Duties:
- Provide initial assessments, treatment planning, and follow up care for clients of The Center who experience Serious Emotional Disturbance (SED), Severe and Persistent Mental Illness (SPMI), Severe Mental Illness (SMI) and/or Co-occurring Disorders (CODs)
- Participate in a team approach for treatment planning and provision
- Provide education to clients on symptoms, medication effects and side effects
- Coordinate services with other community providers when necessary
- Provide injections to specific clients as prescribed
- Provide consultation to other Center providers to assist in crisis intervention
- Ensure that all clinical services provided comply with clinical guidelines, ethical standards and regulatory bodies
- Understand and achieve productivity and efficiency goals established by The Center for the position
- Produce timely documentation
- Adapt to changing work priorities and a fast-paced environment while maintaining professionalism
- Utilize Evidence Based Practices and Trauma-Informed approaches
Required Qualifications
- Background void of issues related to abuse or neglect
- Valid Kansas driver's license with favorable record
- Licensed by Kansas State Nursing Board as Nurse Practitioner with Certification as a Clinical Nurse Specialist or as an Adult Nurse Practitioner through the American Nurse Credentialing Center baccalaureate and Master's in nursing
- Completion of practicum during master's program in a psychiatric setting with training in diagnosis, prescribing, and following patient care
