Purpose of Your Job Description
Responsible for assisting the Director of Health Services in oversight and management of the Health Services Department. As the Clinical Care Manager, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
The RN Clinical Care Manager responsibilities are but not limited to:
Responsibilities

Assist Director of Health Services in oversight and management of Health Services Department
- Ensures member care per Title 22

Ensures department compliance with Title 22 in all RCFE aspects

Assist in scheduling and staffing of Health Services Department

Provides Coverage of Department in absence of Director of Health Services

Assist in preparation for annual surveys.

Provides assistance of clinical personnel supervision in Health Services Department
-
Assist in development and implementation of employee training per Title 22 annual in-service requirements

Assist in monitoring employee training and certification/ licensing requirements
-
Assist in oversight of medication program and med- pass training per current Title 22 medication guidelines

Assist in new employee orientation process per Title 22 requirements

Assist with potential and existing Member assessments
-
Assist with Wellness Clinics and annual influenza clinic coordination
-
Provide input into Health Services employee evaluations per company policies
-
Meets the quantitative performance standards. Consistently meets deadlines.

Makes decisions, prioritizes and carries out daily responsibilities accordingly. Acts with a sense of urgency.
Daily Functions
-
Participating in daily stand-up meetings and other meetings deemed necessary by Director of Health Services

Communication and care coordination with health care professionals
-
Audits of medication cart, pass and MARs
-
Frequent update and review of Member charts, care plans and physician’s reports
-
Supervision of floor and Health Services staff and clinical personnel and their daily assignments
-
Monitoring and documenting Member health and skin conditions
-
Communicating with and responding to Member and Family needs, concerns and grievances
-
Med pass, documentation and management including monitoring Members for adverse reactions to medications and treatments

Assist with community tours and inquiries as needed

Coordinate Member care with other departments within community

Reporting Member concerns to Director of Health Services

Attend care plan meetings and implement changes
Specific Requirements
-
Excellent communication skills both written and oral
- Excellent customer service skills

Ability to educate and train employees
- Medication technician training
- Experience in skin assessments and wound care
-
Bilingual in languages other than English is a plus

Education and Experience
- Registered Nurse license
- Knowledge of Title 22 as it pertains to the RCFE setting
- 2 years in Assisted Living and Long Term Care
- 1 year or more experience in Geriatric Care

1 year or more experience in Alzheimer’s and Dementia care preferred

BLS and First-aid certification
-
2 years in Supervisory role
