Job Summary:
As our QA and Medical Records Specialist, your focus will be on maintaining and upholding current Plan of Care for each client, ensure that Physician Orders are completed accurately, and high-level medical charting for all documentation, including those submitted to the Department of Labor, is complete.
Responsibilities:
- Overall assessment and maintenance of Client Plan of Care, which includes reviewing and editing nurse documentation specific to client 60-day recertification, monitoring for accuracy and compliance with applicable regulations, formatting and creating POC document, obtaining all required signatures, and uploading completed POC to client EMR and individual medical records folder.
- Review all Physician Orders, making certain that they are completed accurately and compliant with applicable regulations.
- Communicate with clinicians as needed for corrections and to ensure documentation is compliant with company standards, state regulations and completed in a timely manner.
- Communicate with MD offices as needed to ensure timely signature of Physician Orders and Client Plan of Care.
- Assist with educating clinicians on documentation and related process improvements.
- Update and maintain QA Plan of Care tracking sheets for each client
- Analyze common trends and report out monthly on QA trends tracking sheet.
- Monthly chart audits to verify POC is uploaded and current, MD orders complete and signed, and review of random nursing Head to Toe Assessments to ensure high-level medical charting standards and policies are being consistently followed in documentation.
- Maintaining accurate and up-to-date client medical records
- Obtaining medical records for prospective and current clients
- Managing incoming faxes and uploading to specific client folders
- Remain informed of home health regulations and standards.
- Provide updates on progress and any issues or assistance needed to QA Manger weekly and/or as needed.
- Other related duties may be assigned.
Qualifications:
- Registered Nurse for at least two years.
- Equivalent experience reviewing clinical documentation for compliance.
- Preferred home healthcare experience
- Understanding of Home Health documentation regulations and best practices

- Full time remote Monday-Friday 9:00am-5:00pm MST
- We offer a competitive compensation of $65,000-$80,000 annual salary, health benefits, unlimited PTO, 401k
