We are seeking a skilled and compassionate Family Nurse Practitioner (FNP) with experience in both primary and urgent care. This position requires proficiency in managing patients across all age groups, from newborns to geriatrics. The ideal candidate will demonstrate clinical expertise, excellent communication skills, and a commitment to delivering high-quality patient care in diverse community settings.
Responsibilities:
- Conduct comprehensive physical assessments, diagnose and treat common acute illnesses and injuries in patients of all ages.
- Manage chronic health conditions, develop treatment plans, and provide ongoing care and education to patients and their families.
- Perform routine health screenings, immunizations, and preventive care services.
- Order and interpret diagnostic tests, such as laboratory tests, X-rays, and EKGs.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and referrals as needed.
- Maintain accurate and up-to-date medical records using electronic health record (EHR) systems.
- Provide patient counseling and education on health promotion, disease prevention, and lifestyle modifications.
- Participate in quality improvement initiatives and adherence to regulatory standards.
- Stay current with evidence-based practice guidelines and continuing education requirements.
Qualifications:
- Master's or Doctorate degree in Nursing (MSN/DNP) from an accredited Nurse Practitioner program.
- Current national certification as a Family Nurse Practitioner (FNP).
- Active NY state license as a Nurse Practitioner with prescriptive authority.
- Minimum of at least 2-3 years of clinical experience as a Nurse Practitioner, with primary care and urgent care experience preferred.
- Comfortable treating patients of all ages, including pediatrics, adolescents and geriatrics.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a multidisciplinary healthcare team.
- Proficiency in using electronic health record (EHR) systems.
