**Overview:** We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide high-quality care to patients in the comfort of their homes. **Duties:** - Conduct in-home patient assessments and develop individualized care plans - Administer medications, treatments, and wound care as prescribed - Monitor patient vital signs and condition, reporting any changes to the healthcare team - Educate patients and their families on proper care techniques and disease management - Collaborate with physicians and other healthcare professionals to ensure coordinated patient care - Document patient progress and treatment plans accurately in electronic medical records **Skills:** - Proficient in medical terminology and documentation practices - Experience in a hospital, ER, or managed care setting preferred - Knowledge of infectious disease care protocols - Ability to work with geriatric patients and those with behavioral health needs - Understanding of HIPAA regulations and patient confidentiality - Strong interpersonal and communication skills Joining our team offers the opportunity to make a meaningful impact on patients' lives while enjoying a supportive work environment that fosters professional growth and development.
