The Resident Care Manager is primarily responsible for supervising, implementing, coordinating and managing skilled nursing resident care through interpersonal contact with residents, families, nursing and ancillary team members. Both care management and supervisory responsibilities must be executed in accordance with state and federal regulations and facility policies and procedures.

Primary Responsibilities:
- Serve as a direct supervisor to skilled nursing assistants, participate in hiring, counseling and termination activities and decisions.
- Monitor resident care needs and documentation, identify and respond in a timely and appropriate manner to changes in condition.
- Monitor the performance and documentation of nursing staff in caring for residents and assure resident rights are honored and nursing services are provided that allow residents to attain or maintain their highest practical level of physician mental and psychosocial well-being.
- Take primary responsibility for monitoring and responding to resident skin conditions.
- Coordinate care with other health care professionals, physicians, social workers, therapists, mental health providers, dentists, dental hygienists etc… to ensure resident are plans are carried out.
- Work collaboratively with social services on admission, transfer and discharge of residents including meeting the needs of residents residing in the independent living area of Heron’s Key
- Assist with reviewing appropriateness of new admissions, inputting physician orders, ensuring medications are ordered and available for residents and assist in developing the resident care plan.
- Work collaboratively with the MDS Nurse manager to ensure documentation in the chart necessary for the MDS is accurate and timely.
- Participate interdisciplinary team meetings including clinical team meeting, Stand-up and PDPM review meetings.
- Participate in nurse management weekend rotation on-call duties and nurse management daily rounds including monitoring of the 24 hour report..
- Participate in chart audits as assigned by the Director of Nursing to ensure Federal, State and local regulations are met.
- Respond in a leadership capacity to emergency situations related to patient and staff safety including active participation in emergency drills .
- Assist with quality improvement activities (QAPI) including surveillance, problem identification and resolution.
- Exhibit excellent client service and a positive approach toward residents, families, staff, visitors and vendors.
- Communicate and function productively with members of the interdisciplinary team, residents, associates, visitors, government agencies/personnel etc.. under all conditions and circumstances.
- Participate in meetings and interdepartmental projects as assigned.
Required Knowledge, Skills and Experience:
- Knowledgeable in nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to skilled nursing facilities and long term care.
- Possess leadership and supervisory abilities, and a willingness to work harmoniously with both professional and non-professional personnel.
- Able to plan, organize, develop, implement and interpret the programs, goals objectives, policies and procedures, etc., necessary for providing quality care.
- Patience tact, cheerful disposition and enthusiasm, as well as handling difficult residents
- Willing to seek out new methods and principles and to incorporate them into existing nursing practices
- Able to relate information concerning a resident’s condition.
- Able to keep information confidential and abide by HIPPA regulations.
