Healthcare Staffing Professionals has an immediate need for a Medical Case Worker, Navigator (Program: Division of HIV/STD). The Medical Case Worker I (Re-entry Navigator) position is responsible for interviewing clients, discharge planning, linkage to medical services, navigation support in the field, and collaborating with other key partners.
The Re-entry Navigation program will identify HIV-positive clients within the LA Sheriffs Department jails and provide intensive linkage services upon release. Navigators will be responsible to build rapport with the client during incarceration period, reach agreements for medical linkage plans, coordinate with internal programs (Custody PHIs, LRP, etc.) and external partners (CHS, ODR, etc.), maintain contact with the client, review medical records, continually update release planning, provide client support when released, locate client in the field to connect client with HIV medical specialty services and medical case management when possible, and help resolve immediate needs.
This position requires driving to work locations, including correctional settings, client residences, clinics and other providers, and to DHSP headquarters. The Medical Case Work must become proficient in working with persons of high acuity with incarceration history, in a trauma informed care approach. The Navigator will liaison with a broad base of providers to ensure that the clients needs are met with the aim of improving health outcomes.
Navigators maintain updated knowledge in local HIV/STI resources and educate and prepare the client to access services. Navigators will locate clients in the field and will use safe locating techniques to meet in-person and use transportation services for initial medical appointments. This will include driving County Vehicles to transports patients. Navigators will collect and maintain confidential health and locating information, accurately enter data, and report updates to the supervisor during one-on-one case reviews or group case conferences.
Position will start upon acceptance and employment clearance and is initially funded through 6/30/2025

$28.71/hr

Minimum Qualifications:
- One year of experience providing services to patients and clients in hospitals, clinics, rehabilitation, residential, or health care facilities. -OR-
- Bachelor's degree from an accredited college or university and one year of professional casework experience interviewing, counseling, and assisting patients/clients with social problems.
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
