About Us: Fathers’ UpLift is the country’s first mental health and substance abuse treatment facility for fathers and families, helping fathers become and remain emotionally stable for their children. Fathers' UpLift uses peer coaching, father-child therapy, training, youth programs, and support for incarcerated and recently released fathers to honor and rehabilitate the relationship between thousands of fathers and families in the Boston area. In their outpatient clinic, over 800 fathers and families receive mental health services from clinicians trained in trauma who reflect the population being served. Outside of the clinic, Fathers' UpLift partners with community centers, substance abuse programs, and other agencies to build a holistic approach to helping fathers rebuild their lives and their relationship with their families.
General Summary: The Intake Triage Clinician is responsible for providing high-quality customer service, assessment, and triage services to clients who are seeking coaching, clinical, and/or group services at our clinics. The clinician explains our service offerings and availability to prospective clients, conducts clinical intakes as necessary, and ensures that the client is referred to appropriate follow-up services within the clinic, higher or lateral care, using superior assessment, diagnostic and triage skills. The Intake Clinician will also provide direct clinical services. The ideal candidate must possess communication skills, active listening, assessment and problem-solving skills, quick thinking and analysis. The Intake Clinician will also possess experience providing individual clinical service.
Knowledge & Characteristics
Principles, practices, and methods of community mental health treatment including interviewing, assessment, marriage, family, child, and group counseling and crisis intervention;
Adult and child psychology, including development and psychopathology;
Knowledge of substance use and mental health.
Applicable city, state, and federal guidelines and laws affecting mental health treatment, services, and reporting procedures;
Characteristics and types of mental and emotional illness;
Principles of psychological growth and development;
Community mental health resources and services and other health, financial, social, recreational cultural, housing, and legal advocacy services.
Essential Responsibilities of The Participant as part of Intake Team:
Greet walk-ins when applicable and direct them to the Patient Representative(s)
Maintain a clear understanding of clinic’s services and their availability, including clinical services, coaching, and groups, such that they can communicate offerings to prospective clients
Direct the flow of prospective clients from the Patient Representative(s) to service providers
Help prospective clients complete their intake paperwork when applicable
Conduct intake/assessment appointments with prospective clients
Assign prospective clients to the appropriate coach
Track and update provider caseloads
Assign prospective clients to groups and track group rosters
Facilitate existing clients connecting to new services
Manage waiting lists, where applicable
Track efforts is EHR database
Provides direct and indirect mental health services in the outpatient clinic and with various community agencies and groups; functions as a member of a multidisciplinary team for the handling of scheduled and unscheduled clients; performs an initial client intake and assessment and develops and carries out treatment plans and makes referrals for a wide range of assigned child and adult clients, including case management.
Complete required Wellness Check-in and Re-Evaluations.
Provides individual, group, and family therapy and crisis intervention to those experiencing life crises and severe mental disorders;
Accepts "on duty" assignments for emergency screening and follow up on unscheduled clients;
Evaluates and signs commitments for clients requiring hospitalization; provides evaluation and referral for court defendants;
Attends case conferences to review client cases and resolve difficult therapeutic problems;
Maintains detailed client records and prepares a variety of reports for review;
Consults with and serves as a liaison with community agencies and other mental health programs and services to evaluate community health and social services topics and issues;
Help shepard potential clients who are currently incarcerated through the intake process, including on site at their place of incarceration, as needed
Essential Responsibilities of The Participant as a Clinical Provider
Assessing and evaluating client mental health status, developing effective treatment plans, and making appropriate referrals;
Providing ongoing therapeutic support and counseling for individuals, groups, and families;
Coordinating treatment with other community agencies and services;
Performing crisis intervention as necessary;
Providing case management services to the chronically mentally ill;
Preparing complete and detailed documentation of client visits and other required reports;
Maintaining organized and accurate records conferences and charting audits and approval of client treatment plans;
Conducting mental health educational programs and workshops;
Identifying the cultural problems affecting mental health practices;
Establish and maintain effective working relationships with other clinicians, city staff, Child Welfare staff, community health or other referral agencies, and the public.
Qualifications
Degree: Master of Social Work or Master in Psychology (or similar)
Experience working with youth and families, especially youth and families of Color
Strong communication skills, both verbal and written. Must be computer literate (working knowledge of word processing, PowerPoint, Excel).
Self-motivated, good organizational and interpersonal skills, detail-oriented, ability to multitask, and prioritize.
Ability to ensure the confidentiality of fathers, youth, and families.
Enthusiasm for working with fathers, youth, and families.
Ability to work with high pressured callers and potential clients
Excellent verbal and written communication skills

Appropriate patient diagnosing

Experience working with Youth and/or clients with a SUD diagnosis a plus
