The Integrated Behavioral Health Provider within Chatuge Family Practice works collaboratively with providers and other members of the healthcare team to improve the mental health of all patients, as well as those struggling with addiction and co-occurring disorders. The position provides individual, family, and group therapy services by utilizing a variety of therapeutic approaches, including motivational interviewing. The Integrated Behavioral Health Provider facilitates therapy services by conducting assessments, providing treatment recommendations, creating service plans, coordinating services, addressing barriers to care, and promoting recovery.
In collaboration with the practice Physicians, Nurse Practitioners, and other clinical professionals such as MAHEC for Care Coordination, this position is responsible for behavioral health assessment and intervention, resource connection, addressing barriers to care and treatment, treatment adherence, and monitoring among others. The essential job functions include:
· Providing brief behavioral health support to persons living with addiction, as well as other mental health disorders
· Provides individual, family, group, and other counseling services.
· Monitoring adherence to medical and behavioral health treatment
· Connecting patients with community resources for biopsychosocial, financial, and other needs
· Communicating/collaborating with medical, interdisciplinary team staff, patient/family, insurance companies, and social agencies
· Providing timely documentation of all services within the clinic’s electronic medical record
· Participating in patient care conferences as appropriate
· Assessing coping mechanisms and current functioning in the following areas: life skills, social skills identity, interpersonal, family, and group dynamics
· Planning and developing group and individual treatments as a member of a multidisciplinary team
· Implementing group, individual and family treatments ensuring a safe, comfortable therapeutic environment
· Ensuring the follow-through of treatment goals concerning individual group therapy
· Documenting patients’ progress and response to treatment promptly
· Providing brief intervention using motivational interviewing techniques to individuals with identified needs who are living with chronic illnesses.
Minimum Qualifications
· Licensed Clinical Social Worker (LCSW)
· Prior relevant work experience
Preferred Qualifications
· Licensed Clinical Addictions Specialist (LCAS), but consideration for Licensed Addiction Counselor (LAC) with at least one year of experience.
· One-two years of relevant work experience
· Certificate/Training in Motivational interviewing
· Certificate/Training in Substance Use Disorders
· Certificate/Training in Dialectical Behavioral Therapy
· Certificate/Training in Trauma Informed
· Certificate/Training in Cognitive Behavioral Therapy
· Certificate/Training in Pharmacology I and II (directly from CAT Courses to obtain LAC)
· Certificate/Training in Principles of Addiction Treatment (directly from CAT course to obtain LAC)
Physical Requirements:
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc. Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items. Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.). May be expected to stand in a stationary position for an extended period of time.
