Primary Nurse Care Coordinator Activities
This is an office position and not a remote position.
o The RN Case Manager acts as a liaison between patients and health care providers to ensure patients are receiving services.
o Provide supervision and support to the Certified Medical Assistant and Nurse Aide.
o Attend all required agency meetings.
o Attend all trainings and meetings in regards to the youth on case load.
o Complete monthly health assessments of all youth in their homes.
o Responsible for ensuring medication management procedures are adhered to and adjusted as needed for all 7 CCC Group Homes.
o Maintain medical files, logs, documentation, etc.
o Creation and maintenance of Medical Passport if one does not accompany youth to placement.
o Coordinate medical services for youth.
o Coordinate and attend all medical appointments as required or deemed necessary.
o Collaborate with CCC Leadership Team members to coordinate medical care for youth when deemed necessary.
o Complete MCO documentation and provide input for treatment plans.
o Ensure EMR documentation is completed timely and according to set standards.
o Ensure youth have necessary medication in a timely manner:
o Calling in Medications
o Picking up Medications
o Distributing Medications to Group Homes.
o Creation and maintenance of MARs (Medication Administration Records) for all youth in the CCC program.
o Conduct audits of group homes in regards to medication management as deemed necessary.
o Complete monthly menu and disseminate to group home staff.
o Conduct monthly training of Medication Administration Training (8-10 hours) in the evening.
o Serve after hours on-call per schedule (additional pay provided).
Additional Duties
o Attend training and development deemed necessary for this position.

QUALIFICATIONS
· RN or greater clinical licensure. BSN is preferred.
· Minimum of 5-7 years of experience is preferred.
· Experience in psychiatric, residential, and/or adolescent care is strongly preferred.
· Experience with psychotropic medications is strongly preferred.
WORK ENVIRONMENT
This job primarily operates from the Lexington Office. The Nurse Care Coordinator will be required to travel to the group home settings as deemed necessary in order to carry out all job duties effectively. Employee must be able to travel to our three group home locations: Lexington, Nicholasville or Georgetown. A work cell phone and laptop will be issued for this role.

While performing the duties of this job, the incumbent is regularly required to talk or hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Pay: $65,000.00 - $70,000.00 per year
