Works under the supervision of the Manager, Outpatient Treatment Unit and collaboratively with staff and physicians . Assesses and evaluates patient needs and provides care to inpatients and outpatients requiring wound, ostomy or continence management with knowledge of regulatory standards. Promotes consistency in policy/procedure, supplies, and education material. Able to perform patient care with a case management approach. Demonstrates clinical and theoretical education expertise to serve as role model for personnel.
- Wound Certification required within 18 months of hire*
Pay Range: $28.71 - $48.77/hour
Employment Type: Part Time Days, 40 hours every 2 weeks
Schedule: Week 1: Monday and Friday 7:30 am - 4:30 pm
Week 2: Monday, Tuesday, and Friday 7:30 am -4:30 pm
Requirements: Graduate of accredited Registered Nursing Program required, Current Michigan RN License required, Current BLS for the Healthcare Provider (or completed within 60 days of hire) required
Provides support for the Wound Care program and services
Responsible for support and consistency of Wound Care program policies and procedures and for product evaluation and recommendation in regards to cost- effective and efficient wound, ostomy and continence care.
Works collaboratively and communicates effectively with all members of the Health Care Team to ensure optimal patient outcomes
Provides consultation to physicians and nurses, including patient assessment, care plan development, evaluation of care and discharge planning.
Serves as a clinical resource and mentor to the health care team members. Displays leadership in behaviors and practice.
Clinical Practice and Case Management
Provides comprehensive assessment and care in a holistic and systematic way to In patients and out patients.
Coordinates with other healthcare professionals to meet specific patient needs.
Participates in patient family education and patient care conferences as needed.
Assists staff and physicians with appropriate documentation of care and education.
Promotes continuity of care using patient plan of care and written instructions and makes referrals as indicated to community services.
Delegates and coordinates patient care activities to optimize the delivery of treatment modalities.
Education
Provides education to members of the health care team related to Wound and Ostomy care
Collaborates with the Wound Care Coordinator and clinical educators to support education plans to assure appropriate integration of clinical practice standards throughout the organization.
Provides formal and informal staff education on an ongoing basis
Quality improvement/Best Practice
Participates in performance improvement activities to enhance clinical and operational effectiveness and efficiency
Assists clinical managers, Wound Care Coordinator, and educators in identifying and implementing outcome studies, research initiatives and process improvements to ensure ongoing evaluation of all wound, ostomy and continence care programs.
Assures compliance with practice standards.
Provides support in project improvement initiatives.
