We are looking for a Care Coordinator or two to join our team they must have a CNA and also has their delegation certification (or be able to obtain it within one week of hire) Help coordinate the care of our patients and employees. The ideal candidate will have experience in healthcare and be able to provide comprehensive care coordination services to ensure our patients and employees receive the best possible care and help. The Care Coordinator will be responsible for assessing patient needs, coordinating with other healthcare providers, developing treatment plans, and providing education and support to patients and their families.
We are looking for 2 care coordinators.
We will train the right candidate who has CNA experience. Prior management experience is helpful but not required. Must be able to lead other caregivers and cover a shift on short notice if there is a call off.
There is room for advancement as well. Some of the duties are as follows:
- Coordinate and monitor patient care services.
- Knowledge of Matrix care is helpful but not mandatory.
- Assess patient needs and develop care plans accordingly.
- Communicate with other healthcare professionals to ensure the best possible care for patients.
- Monitor patient progress and adjust care plans as needed.
- Manage a large pool of caregivers and provide discipline if necessary.
- Ensure that all medical records are up to date and accurate.
- Provide emotional support to patients and their families.
- Educate patients and their families about health conditions and treatment options.
- Must be able to fill in for shifts if they need to be covered anytime day or night while on call.
- Maintain regular attendance at the office to execute job responsibilities.
- The ideal candidate would be willing to do marketing. (It's ok if you have no experience because we can train you!)
