This is a full-time RN position that will provide on-call support, emergency after hour services to Big Bend Hospice patients. This is an on the go, spring into action position. Must be willing to perform visits in the patients' homes after hours during the week and on the weekend.
Great Schedule: This position works a (3) 12-hour shift schedule.

Main Duties and Responsibilities:
(Includes, but not limited to)
Provides quality patient care and manages direct care to patients and families as part of the Interdisciplinary Team [IDT], incorporating appropriate nursing intervention and follow-up.
Provides palliative nursing care to patients and families by applying appropriate nursing interventions that incorporate physical, psychosocial, spiritual, and cultural components consistent with the established patient/family plan of care as developed by the IDT.
Revises and updates the patient/family plan of care in response to changing patient and family needs collaborating with other members of the IDT to determine covered services that are consistent with the patient/family goals of care.
Completes all clinical documentation consistent and compliant with the state nurse practice act, best practice hospice professional nurse standards, licensing/regulatory guidelines, and organizational policy/procedures at the point of care as appropriate.
Communicates effectively with patients, families, physicians, vendors, pharmacies and/or facility staff to enhance the coordination of patient care.
Demonstrates clinical skills regarding case management, application of Big Bend Hospice [BBH] policy and procedure, intravenous skills, pain medication titration, symptom management and patient/family relationships.
Demonstrates competence in a variety of patient visit types and care situations (on-call visit, death event, continuation of Continuous Home Care, admission, discharge, transfer, revocation, General Inpatient).
Provides and reinforces family/caregiver education designed to enhance their comfort and independence in providing patient care and support.
Willing to provide patient care in adjacent counties as situations warrant.
Performs other duties as requested.
Education and Experience:
Bachelor of Nursing Degree required.
3 years of nursing experience, preferably in hospice or home health.
Hospice/Palliative Care experience required; CHPN preferred.
Valid driver's license and proof of insurance are required.
