Overview: The Behavioral Health Clinical Case Managercoordinates services to individuals re-entering the community from incarceration or being released from specialty courts. Case Manager will provide comprehensive and individualized substance abuse and mental health treatment, job skills, and life skills to clients in the community.
CLINICAL CASE MANAGER PRIMARY DUTIES:
- Provide mental health and substance abuse evaluations, intakes and assessment, problem identification, and crisis interventions; assess the need for on-going treatment or referral to other agencies
- Utilize CareLogic Caseload Contact Reports to identify and contact clients who are struggling to attend treatment consistently
- Provide outreach to clients if they are not attending treatment regularly and provide ongoing communication to their referral source
- Conduct group counseling sessions at approximately 5 groups weekly
- Collaborate with clients to develop, implement, and revise individualized case management treatment plans and monitor progress toward case management goals.
- Help clients develop achievable goals and objectives within their Individual Treatment Plans that address all relevant issues: income, employment, children’s needs, education, health, mental health treatment, substance abuse treatment, housing, recovery, and legal needs
- Participate in program, agency, and external meetings and trainings.
- Respond to crisis and emergency calls
- Act with utmost professionalism, responding promptly and respectfully to all inquiries, suggestions, requests, and complaints in accordance with agency policy
- Maintain appropriate boundaries with participants, observing all confidentiality and HIPAA protocols
- Maintain excellent written records and required statistical forms, as well as administrative paperwork such as time sheets, mileage sheets, expense reports, and disbursement requests
- Report to assigned Behavioral Health Program Manager
CLINICAL CASE MANAGER QUALIFICATIONS:
· Training in trauma-informed care, harm-reduction, de-escalation techniques, and vicarious trauma
· Excellent communication and computer skills
· Experience working with justice-system programs
· Master’s Level education in the field of social work, criminal justice, or counseling. A Licensed Professional Counselor or Licensed Clinical Social Worker with a Master’s Degree from an accredited university is strongly preferred. CAS/CAT/LAC preferred.
· Experience, knowledge of, and sensitivity to homeless issues, and the particular issues and barriers faced by women and women veterans in homelessness.
· Ability to apply positive engagement, motivational, and leadership skills in a culturally appropriate manner, working with adults from diverse and challenged backgrounds.
· Verbal and written communication skills, including detail-minded record-keeping and excellent computer skills.
· Be able to work independently as well as part of a team, with good personal boundaries and appropriate use of authority
· Ability to hear and process critical feedback, as well as, provide it to others
· Ability to meet deadlines and follow through on commitments.
· Flexibility, creativity, positive attitude, and good humor
· Valid driver’s license and reliable transportation
· If offered position, must pass required TB screening, criminal background check, and have a driver’s license and vehicle.
CLINICAL CASE MANAGER OTHER REQUIREMENTS:
· Deep understanding of and passion for behavioral health and homelessness services
· Compassion for the clients that MHBHC serves and a connection to the mission
· Proof of automobile insurance and clean background check required
· Some travel (within Aurora and the Denver Metro area)
· Evening and some weekend work may be required
· This position will start with a required 90-day probationary period
· Knowledge of MHBHC policies and procedures and ability to enforce them
· Excellent rapport with the client population
· Demonstrated ability to constructively work with persons in crisis situations
