Job Title: Person-Centered Case Manager

Summary:
We are a professional guardianship company seeking a compassionate and experienced Case Manager to provide comprehensive support and advocacy for clients with memory impairments (such as dementia and Alzheimer's) and developmental disabilities. The ideal candidate will have a demonstrated understanding of these conditions and a passion for ensuring the safety, well-being, and highest quality of life for our clients.
Responsibilities:
Client-Centered Care Planning: Develop and implement individualized care plans that address the unique needs and preferences of each client, focusing on their strengths, abilities, and goals.
- In-Person Client Visits: Conduct regular visits to clients' homes, residential facilities, or other care settings to assess their well-being, monitor care plan implementation, and identify any emerging needs.
- Care Coordination: Collaborate with medical professionals, therapists, social workers, and other service providers to ensure coordinated, seamless care delivery and advocate for clients' best interests.
- Crisis Intervention: Respond to urgent situations and provide support to clients and their families during times of crisis or change.
- Documentation and Reporting: Maintain accurate and detailed records of client visits, care plan progress, and all relevant interactions. Prepare comprehensive reports for the court and other stakeholders.
- Advocacy: Act as a strong and effective advocate for clients, ensuring their rights are protected and their voices are heard in all decision-making processes.
- Resource Navigation: Connect clients and their families with appropriate resources, services, and community support to meet their diverse needs.
Qualifications:
Education and Experience
- Bachelor's degree in social work, psychology, nursing, or a related field preferred.
- Minimum of 2 years of experience in case management, social work, or a similar role working with individuals with memory impairments or developmental disabilities.
Knowledge and Skills
- Demonstrated understanding of dementia, Alzheimer's disease, and developmental disabilities.
- Strong interpersonal and communication skills, with the ability to build rapport with clients, families, and professionals from diverse backgrounds.
- Excellent organizational, time management, and problem-solving abilities.
- Proficiency in Microsoft Office Suite and electronic health record systems.
- Valid driver's license and reliable transportation for client visits.
Additional Desired Qualifications:
- Certification in case management (CCM) or a related field.
- Experience working in a guardianship setting.
- Fluency in a second language (especially Spanish).
