As Esperanza’s Program Coordinator, you will play a pivotal role as a liaison between our Veterans, Case Managers, and VA Staff. Your responsibilities are not just tasks, but instrumental in ensuring clinical programming compliance and supporting the Clinical Director by providing training, charts, and collaborative documentation are completed and submitted promptly.
Principal Duties:
· Ensure that intake and exit documentation is complete, submitted, and filed per our agreement with the VA.
· Update the Resident List and ensure billing information is up-to-date and accurate.
· Assist the Clinical Director with program evaluation, mentorship, and staff training.
· Audit Resident files for completeness and best practice.
· Respond to client-related emergencies as needed.
· Support a Veteran-centered, trauma-informed team culture.
· Inputs data within the Homeless Management Information System (HMIS) and regularly prepares for and attends chronic case conferencing meetings.
· Attends team meetings and participates in case conferencing.
· Participates in ongoing training and professional development opportunities.
· Ensure unit inspections are completed bi-weekly or more often as needed.
· Anticipate program needs and take initiative to solve program problems.
· Build and maintain good working relationships with partner organizations.
· Provide case management services to assigned Veterans.
· Other duties as assigned.
Essential Training/Certifications
· Master’s degree in social work, licensed and in good standing with the Board of Social Work.
