The Licensed Clinical Social Worker is an integral part of the Clinical team at Transformative Healthcare Solutions Health Management. The Licensed Clinical Social Worker is responsible and accountable for providing Quality Care, Customer Experience, and Stewardship by coordinating psychosocial care and services for assigned patients in collaboration with the patient, family, and patient care team. The Licensed Clinical Social Worker will practice independently in the provision of services, engage in professional work that relates to behavior, personality, interests, emotions, and mental disorders, consulting with peers and supervisors as appropriate. He/she will provide caring and compassionate behavioral health care to residents within skilled and assisted living facilities, in addition to home and in-office settings. The individual in this role must be able to move at a very fast pace and have a high ability to organize and prioritize tasks and information based on a rapidly changing and growing healthcare environment.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
· Provides specialized services at all levels of complexity for patients with various issues, providing mental health prevention, assessment, treatment, management, and professional consultation services as a mental health provider.
· Promotes the delivery of evidence-based mental health services and interventions for reducing behavioral and psychological symptoms.
· Provides screening for cognitive deficits to address specific questions related to functional ability.
· Provides a full range of empirically supported interventions and approaches that are specific to the geriatric population and chronically mentally ill patients.
· Provides psychological prevention services to clients at significant risk for psychological problems.
· Provide consultation to medical providers regarding the behavioral health components of a patient’s presentation and develops collaborative treatment plans to address symptoms and enhance functioning.
· Assists in developing and maintaining procedures and protocols to enhance behavioral health program relevance and effectiveness for patients and facility staff.
· Utilize an electronic documentation medical health record platform, in which patient note documentation must be completed within 48 hours.
REQUIRED EDUCATION/ EXPERIENCE:
· State Licensed Clinical Social Worker
· Credentialed Medicare and Medicaid (preferred but will assist if needed).
· Valid State Driver’s License.
· Proof of medical malpractice insurance within the company’s required coverage limits.
· Knowledge of state regulations and standards related to client treatment, patient rights, and client/ patient confidentiality.
· Possess proficient ability and experience in computer applications, specifically electronic medical records (EMR) systems.
· Possess proficient computer application abilities to record time, obtain work directions, and complete assigned duties.
· Proficient in Microsoft Office Suite, including Excel, Word, Outlook, and PowerPoint.

Pay: $75,000.00 - $90,000.00 per year
