Responds to needs of long-term mentally ill adults who participate in case management activities of CMCSB. Duties include planning the delivery of case management services to meet assessed needs, linking individuals to planned services and resources within the community, coordinating services and plans with collateral agencies and providers, monitoring individuals’ progress towards identified service plans and goals, advocating for needed resources and services for individuals, providing basic counseling services, making home visits to individuals receiving services, providing information and education to individuals receiving services regarding psychiatric illnesses, performing all administrative functions necessary to carry out the above services with an emphasis on records documentation and the timely submission of required service reports, managing aggressive behavior of individuals receiving services utilizing approved techniques, participating in continuing education programs to enhance professional competency, and transporting individuals receiving services to needed services and related functions, as necessary.
