Health Advisor and Client Care Manager – Integrated Advocacy, Private Nursing and Home Care

Broad Street, a Premium, Innovative Home Care Solutions Provider seeks an experienced Nurse, Physical Therapist or Occupational Therapist to serve as a Client Care Manager and Patient Advocate. This is a great role for a Nurse or Therapist who wants to transition their clinical knowledge and experience into longer term client relationships. Because we are a Concierge Health Service which is private pay, we don’t have the administrative burden of managing Insurance and the related coding, so we are free to focus on Care. And unlike working in a hospital or clinical setting, where the treatments are more defined, we work towards a solution with a wide range of options and the paths we can take are unlimited. The role specifically includes the following:
- Advise clients on potential care solutions utilizing our combined capabilities, and then continue to advise and adjust to support changing needs and preferences
- Connect and collaborate with clients’ physicians and healthcare network in order to incorporate prescribed regiments into daily routine and to communicate back efficacy to help better inform clinical decisions
- Utilizing our Professional Network of resources, work collaboratively with partners, clinical and non-clinical, to provide seamless support
- Establish health plans and then working with our HR department to identify and introduce Support Professionals (Caregivers and/or Private Nurses)
- Management of Client Care to support a consistent, high-quality Client experience and successful relationship with Support Professionals
- Help build our Brand as an employer, through Relationship Management within Care Teams that promotes teamwork and positive outcomes
Broad Street provides premium, customized in-home care and nursing services for seniors who want to remain healthy and independent in their homes. We offer support to individuals whose lives are impacted by more complex health issues. It starts with our Care Managers who develop a customized care plan and detailed training to our team of Care Professionals.
Knowledge, Skills, and Abilities Required
The Client Care Manager and Patient Advocate must meet the following qualifications:
- Excellent interpersonal and communication skills - oral, conversational, telephone and written with Clients, Care Professionals and Nurses, other Care Managers and other Office Personnel (We are a team)
- Self-Starter – Ability to proactively see a need and take necessary action to find a solution
- Able to assess caregiver and private nurse skills, personalities, and interests to help inform the placement process
- Demonstrated Professionalism
- Team player – ability to step into other roles and support each other on behalf of our clients and Support Professionals
- Direct support experience, to help train direct Support Professionals
- Technically sufficient with the ability to learn new technologies
- Responsive and Accountable – as the primary point of contact and subject matter expert for our clients, we need to be responsive to get the support, particularly in time of need
Experience:
- RN or PT or OT license (Required)
- Clinical experience with specific conditions including Parkinson’s Disease and other Movement Disorders, Alzheimer’s Disease and other Dementias, Stroke, Cancer and other Chronic physical or cognitive conditions (Preferred)
