Unique Hospice is seeking a passionate and dedicated Registered Nurse (RN) to join our team. This position involves direct patient care in the field, focusing on individuals receiving end-of-life care. As part of our interdisciplinary team, you will play a crucial role in enhancing the quality of care and life for our patients.
Responsibilities:
- Provide direct patient care as defined under the State Nurse Practice Act, with a focus on end-of-life care needs.
- Accurately document patient services to ensure they reflect the planned care and are compliant with legal and organizational standards.
- Assess patient and family/caregiver needs, providing education and information relevant to the diagnosis and care plan.
- Participate actively in the coordination of hospice services, reporting needs and adjustments to the interdisciplinary group. This includes attending biweekly interdisciplinary team meetings.
- Conduct RN supervisory and re-certification visits, ensuring timely and efficient care management.
- Maintain competency in nursing and assessment skills, identifying and communicating any changes in patient conditions that may affect the care plan.
- Fulfill other duties as assigned by the Registered Nurse or Case Managers.
Qualifications:
- Must be a licensed driver with an automobile insured in accordance with State of California regulations.
- Strong understanding and embrace of hospice philosophy and the needs of terminally ill patients.
- Excellent prioritization and communication skills are essential.

Pay: $115,000.00 - $130,000.00 per year
