CHARACTERISTICS OF THE WORK
Under close supervision, performs behavioral health assessments in the form of telephone follow-up, monitoring, and support for patients referred by Licensed Clinical Social Workers (LCSWs).
Enhances treatment adherence and clinical outcomes by monitoring patients' response to behavioral health interventions, which may have included counseling by a LCSW and/or medications from a FHC medical provider or the prescribing behavioral health consultant.
Provides supportive services to the LCSWs.

Schedules appointments.
Maintains the community resource list.
Initiates phone calls to community resources to assist referrals.
Organizes patient education materials.
Engages patient in the identification of initial or revised self-management goals.
Assesses patient's current level of functioning by administering appropriate assessment instruments, per the recommendation of a LCSW.
Assesses patient's response to any prescribed medications.
Monitors patient's progress with treatment recommendations and self-management goals.
Schedules follow-up contact with patient, based on results of the most recent contact and assessment of the patient's current level of functioning.
Conducts follow-up with patients, primarily by phone, to monitor progress toward treatment recommendations.
Provides support and assists patients with identifying new self-management goals when appropriate.
Documents as appropriate in the medical record, using handwritten forms and/or a computer to complete required documentation.
Provides verbal feedback to the referring LCSW and primary care physician, as necessary.
Identifies patients with Limited English Proficiency (LEP) and/or other communication barriers and uses appropriate interpretive service; documents use of interpretive services.
Manages medical records documentation, requests and/or releases.
Participates in departmental meetings.
Adheres to standard precautions.
Performs related work as required.
MINIMUM REQUIREMENTS
Bachelor's degree in Social Sciences or a closely related field.
An equivalent combination of training and experience may be substituted, as determined applicable by Civil Service.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
Responds to patient and Family Health Centers' needs via the telephone, internet, fax, computer software and in person.
Identifies medical record documentation, requests and/or releases.
Lifts and moves materials weighing up to 5 lbs.
Spends extended periods of time sitting, standing, walking, climbing, stooping, bending and reaching.
SPECIAL REQUIREMENTS
Must be available to work a varied schedule and/or work overtime as necessary.
Must provide own transportation to other FHC locations as needed.
