The Registered Nurse Navigator, Obstetrics obtains and maintains a patient’s pregnancy encounter throughout the entire pregnancy including post-partum time frame. Serves as a consistent point of contact for patients, reviewing care plan questions they may have, confirming all routine health maintenance testing and education completion.
JOB DUTIES AND RESPONSIBILITIES:
Complete OB Intake appointment - completing all medical history and education required.
Maintain personal EPIC list with all patients that they have completed intake on to track through each trimester.
Maintain practice specific list for OB patients to track transfers in-out, projections, and transfer details.
Complete ONAF documentation three times for pregnancy episode for all state insurance plan patients within state guidelines. Complete FMLA paperwork as requested.
Complete Check-In Calls at 16 week-28/30 week and 1 week postpartum. During the calls confirm labs are up to date, review educational folder contents, ensure patient is up to date for routine appointments at OB and MFM offices and review scheduling for prenatal education courses.
Complete Depression Screening for each trimester and postpartum call.
Complete any coordination of care appointments with specialist during pregnancy, anesthesia and/or NICU.
Review of educational classes offered and documented in flowsheets.
Review community resources-Nurse Family Partnership and place referrals appropriately.
Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
Other duties as assigned.
PHYSICAL AND SENSORY REQUIREMENTS:
Sit up to 7 hours per day; 3 hours at a time. Stand for up to 3 hours per day; 2 hours at a time. Walk 1 hour per day; 10 minutes at a time. Frequently lifting, carrying, and pushing objects up to 10 pounds. Rarely lifting, carrying, and pushing objects up to 75 pounds. Frequently stooping and bending. Frequently reaching above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, high and low frequencies. Must be able to see as it relates to general near, far, color and peripheral vision. Must be able to speak as it relates to normal conversation and projection as it relates to lectures and presentations.
EDUCATION:
Registered Nurse with current license to practice in the Commonwealth of Pennsylvania required. Registered Nurse with current license to practice in both the Commonwealth of Pennsylvania and the State of New Jersey is preferred.
TRAINING AND EXPERIENCE:
Two-years women’s health experience required.
