Inventory Management Assists purchasing technician in maintaining adequate stock levels of all medications and supplies using the established inventory control procedures.
Formulary Cost Management Communicates and demonstrates the purpose and content of the Formulary system. Consults with physicians regarding non-formulary requests as determined by departmental policies and protocols.
Staff and Patient Consultation Consults with and advises other Healthcare professionals and patients on matters pertaining to selection, procurement, distribution, and safe and appropriate use of medications.
Medication Order Review Reviews prescriptions and medication orders, checks for appropriateness and provides information needed to properly and safely administer the medication. Recognizes when orders are inappropriate and takes responsibility to assure that the order is modified promptly and correctly. Documents interventions.
Product Formulation Determines whether extemporaneously prescribed sterile and non-sterile products, not commercially available, should be formulated. Develops the formulation procedures using acquired knowledge of pharmaceuticals and acquired skills.
Records Management -Pharmacy Maintains records such as pharmacy files, inventories, controlled substance records, and registries; ensures records are maintained accurately and in compliance with regulations, policies, and procedures.
Controlled Substance Process Control Manages the disposition of controlled substances and other dangerous drugs in accordance with legal and regulatory requirements. Manages systems of inventory record keeping and control to detect any shortages or discrepancies in controlled substances at the earliest possible time
Monitoring Drug Therapy Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, appropriateness of drug and dose. Monitors patient microbiology and laboratory indices and recommends appropriate dosing changes when necessary. Reports adverse drug reactions and medication errors in the even reporting software.
Clinical Pharmacotherapy Services Provides clinical pharmacy services as outlined in approved hospital policies, including but not limited to, pharmacokinetic consults, renal dosing, parenteral nutrition management, anticoagulation management, neonatal/pediatric/geriatric dosing management, route optimization, antimicrobial stewardship, and reviewing medications at transitions of care.
Continuing Medical Education Participates in scheduled in-services, educational presentations, computer based training, and staff meetings in ensure ongoing understanding of and adherence to professional standards, regulatory requirements and organizational policies and practices.
