SUMMARY_________________________________________________________________________
Involved in training, providing, and documenting direct patient care and services for patients and supervises clinical support care/services/personnel, while working collaboratively with others to achieve established objectives/goals. Effectively performs and manages a variety of job functions, exercising sound decision making principles according to the philosophy, mission, and policies/procedures/protocols of the Organization and rules, regulations, standards, and laws. As a recognized patient-centered medical home (PCMH), every employee is responsible for ensuring the successful implementation of quality, comprehensive patient-centered care as a member of the patients care team. ESSENTIAL DUTIES AND RESPONSIBILITIES (include, but are not limited to, the following):
Clinical Leadership

Function as intake resource expert for clinical staff; organize and provide training to clinical staff during New Employee Orientation

Collaborate with Director of Clinical Services, Quality and Risk Management and Clinical Support Team to provide training, support, and annual staff education to clinical staff and providers

Work with Director of Clinical Services, Quality & Risk Management to coordinate new and existing programs
-
Coordinate training from Providers to Staff members on effective management and implementation of programs
-
Provides community education classes on different topics as needed.

Collaborate with Practice Mangers to order and maintain stock of all necessary supplies in all centers.

Work on template creation for program to communicate with EHR system

Work with QA/QI Nurse Manager to measure Quality Standards to reach benchmarks.
- Ensures implementation of goals, objectives and activities and abides by protocols, as outlined in the grant-related work plans and requirements.

Coordinates with clinical nursing and other staff to design and implement outreach/in-reach and education activities focusing on family planning for clinic patients and community memb

Increases staff expertise in working with targeted populations through in-service training and related activit

Develops tools for public information and outreach and ensure implementation of outreach work plan.

Participates in external meetings as required by funders and networks with other relevant community organizations and coalitions as directed by supervisor

Develops and implements other group education activities and focus groups.
Manages, tracks and reports service delivery and outcomes to meet MedLink's and any grant requirements, including budget management, narrative and statistical reporting HIV/HCV Grant Programs

EDUCATION and/or EXPERIENCE_____________________ ______________________________________
Current unencumbered Georgia RN license. Bachelor's Degree in Nursing preferred.
Experience in ambulatory care, community health care network/clinic, public health, grant or health program coordination experience a plus.
Possesses knowledge and skills of:
Familiarity with Georgia communities preferred
Small group facilitation, curriculum development and program evaluation skills.
Computer skills at the level necessary for working efficiently in the company's practice management system, especially in the electronic medical record; Microsoft office applications, including e-mail, WORD, Excel; on-line social networking
