Job Title/Position: Hospice Registered Nurse Case Manager
Reports To: Clinical/Nursing Supervisor
Job Description Summary
The Hospice Registered Nurse Case Manager plans, organizes and coordinates hospice care and services for the patient and family/caregiver.
Essential Job Functions/Responsibilities
The Hospice Registered Nurse Case Manager provides professional nursing services in accordance with the plan of care and accepted standards of practice including but not limited to:
- Performs initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living, including but not limited to:
A. Risk for pathological grief
B. Cultural and spiritual implications
C. Verbal and non-verbal communication pattern
- Coordinates and implements the individualized plan of care recommending revisions to the plan as necessary, including but not limited to:
- Managing discomfort and providing symptom relief
- Specialized nursing skills related to palliative end of life care
- Consults and educates the patient and family/caregiver regarding:
- The disease process
- Self-care techniques
- End-of-life care
- The processes for dealing with issues of ethical concern
- Initiates appropriate preventative and rehabilitative nursing procedures
- Prepares clinical and progress notes that demonstrate progress toward established
goal(s)
- Coordinates all patient and family/caregiver services and prioritization of needs with
members of the interdisciplinary group
- Utilizes case management approach and referring to other services as needed
- Informs the physician and other personnel of changes in the patient’s needs and
outcomes of interventions
- Determines scope and frequency of services needed based on acuity and patient and
family/caregiver needs
- Supervises LPNs and paraprofessionals providing services to the patient with or without the LPN according to present at least every 30 days
- Participates in in-service programs
- Provides specialized hospice training to other staff, family members and informal
caregivers to insure adequate care
- Evaluates the patient and family/caregiver response to care on an ongoing basis
- Assesses the ability of the caregiver to meet the patient’s immediate needs upon
admission and throughout care
- Evaluates own needs for support and using identified system(s) to meet the need
- Applies specific criteria for admission and re-certification to hospice care to establish
appropriate levels of care and the patient’s eligibility
- Communicates information using current process and technology available to the
organization
- Participates in performance improvement program
- Complies with organization’s policies and procedures
- Performs other duties as assigned
Position Qualifications
- Degree in Nursing from an approved school of nursing as determined by the Board of Nursing, required
- Licensed to practice registered nursing in the state by the Board of Nursing, required
- Hospice or home health care experience, preferred
- Management/supervisory experience helpful but not required
- Must be a licensed driver with an automobile that is insured in accordance with state and/or
organization requirements and is in good working order
- Excellent observation, verbal and written communication skills, problem solving skills, basic
math skills; nursing skills per competency checklist
- Understands the hospice philosophy of care
Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required.
PHYSICAL LABOR REQUIREMENTS
- Functioning auditory, speech and visual skills. Must be self-mobile*

- Daily home visits, weekly office meeting. Requires dependable transportation
