General Functions: Responsible for leading the Behavioral Health Department's Eviction Prevention program for formerly homeless adults with mental health and co-occurring substance use disorders using evidence-based and trauma-informed practices.
Specific Functions: Develop and lead a formal Eviction Prevention program available to all PSH participants in accordance with Behavioral Health policy BH308 and using evidence-based practices of Housing First and Trauma-Informed Care.
Collaborate with Housing Associate Directors regarding ongoing evaluation of effectiveness of housing retention-related documents (e.g. home inspections, move checklists, etc.) and processes. Make suggestions for revisions as needed and regularly communicate any identified related training gaps/needs within the program.
Plan, organize, direct, and supervise the day-to-day aspects of the department's Eviction Prevention program.
Interview, hire, train, evaluate, and discipline staff in accordance with agency personnel policies and procedures; ensure assigned staff are following all eviction prevention protocols, practices and documentation.
Provide training, supervision and guidance to assigned staff about the provision of assertive engagement, motivational interviewing, harm reduction and eviction prevention/housing retention services to persons with serious mental illness or co-occurring substance use issues.
Meet with individuals in the Behavioral Health department's supportive housing program who are new to PSH to discuss housing retention services and assess for eviction risk; meet at minimum monthly with persons on assigned team who are at-risk of eviction due to missed rent payments, lease violations, and/or safety concerns. Develop and review formal eviction prevention plans. Meetings may occur on-site, in the individual's home, or in the community.
Manage the eviction prevention referrals and waitlist, working with the housing retention team to ensure contact is quickly initiated upon submitted referrals; manage program admissions and discontinuations in the agency's database per protocol.
Oversee the creation and implementation of individual housing retention plans for all participants in the Eviction Prevention program.
Manage day-to-day issues/concerns that arise regarding individuals served and escalate to individuals' Lead Case Manager/Coordinator when needed; always communicate issues with supervisor to keep leadership apprised.
Communicate regularly with the Manager of Properties and Landlord Relations and landlords on housing concerns that relate to advocating for an individual to maintain housing.
Complete inspection of unit for individuals who submit a move request form and/or are planning to discharge from the housing program. Work with supervisor and the agency's Buildings & Grounds and Finance departments to have viable issues resolved prior to the individual moving out of the unit.
Visit with individuals served in their homes in accordance with department procedures.
During home visits:
- Conduct home inspections to assess for safety, habitability, and adequate food supply.
- Take photographs in accordance with department procedures and person's consent.
- Provide support and assistance needed to maintain housing.
- Review eviction prevention plans and follow-up.
Based on results of home visits, complete home inspection forms and landlord reporting forms in resident's electronic record within established timeframes and follow-up regularly on outstanding issues.
Oversee and complete the home inspection oversight process and monitoring of home inspections and outcomes completed by other agency staff; follow up with supervisors in accordance with established procedures.
Monitor tracking system and provide formal ongoing notification to individuals who have outstanding rent and loans with Cornerstone.
Assist community housing staff with resident moves, home clean-outs, and wellness checks/follow-ups.
Complete lease violations for CSI-owned properties and adhere to proper protocols to meet and address with CSI tenants.
Participate in ongoing monthly meetings, including but not limited to:
-a housing manager meeting to communicate pertinent concerns regarding individuals in the eviction prevention program
-a meeting with assigned housing team staff to discuss ways to assist in retaining individuals' housing and/or any safety issues that have arisen.
-a clinical supervision meeting to meet billing requirements
-a department meeting
Provide written documentation of services in sufficient detail and quantity in accordance with MRO requirements, individual program plans, agency quotas, and requirements of any other funding or accreditation source.
Complete weekly tracking documentation as required by an individual's funding source (e.g. DMH 220).
Meet or exceed assigned MRO weekly billing quota as determined by the Director of Behavioral Health.
Complete weekly MRO billing audits for all staff members on the team and submit to Coordinator per protocol.
Participate in all agency and department trainings, monthly department meetings, and any additionally recommended trainings or meetings as required by supervisor(s).
Provide telephonic-only crisis intervention services in accordance with the staff rotation on the weekly crisis line schedule. Maximum of two weekly rotations per year. Crisis phone provided.
Maintain ongoing compliance with all agency-related training and certification requirements.
- Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
- Operate agency or personal vehicle in accordance with established policies and procedures.
- Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.
- Other duties as assigned.
Qualifications: Must qualify as a Mental Health Professional (bachelor's degree in social services field or minimum of five years supervised experience in mental health/human services). Minimum two years of prior experience working with co-occurring mental health and substance use populations and working knowledge of Housing First, Permanent Supportive Housing and Eviction Prevention required. A minimum of one year of supervisory experience strongly preferred. Proficiency in the use of computers and typing, and working knowledge of MS Word, MS Excel and MS Outlook applications required. Driving record and auto insurance coverage in compliance with agency policy required. A criminal background check and post offer physical required. Reliable transportation required. EOE.
